Factual error: Noticed this watching season 2, and I assume it was also the case in season 1. The entrance of the Miramar Playa has flags from various countries flying. One of them is the Canadian flag with the maple leaf. Wrong flag. In that period the Canadian flag was the "Red Ensign", or "Dominion Flag." The maple leaf flag wasn't introduced until 1965.
Continuity mistake: When Buzz and Provenza race out of the squad room to follow Rusty in the frantic search for Stroh, Buzz is in shirt sleeves with no tie. However when they arrive at their destination Buzz is wearing a coat and tie.
Caleidoscope - S2-E4
Character mistake: The chess set in Dr. Pierce's home office is set up incorrectly. When the men are set up properly, each player has a light colored square in the right-hand corner. The board as it appears in the show is set up so that the light-colored square is in each player's left-hand corner. This "beginner" mistake puts the king and queen in the wrong positions.
Factual error: A plastic bag of items is found under the footboard. Plastic bags weren't invented in 1969. They first appeared in public in 1979.
Continuity mistake: Before and after the Beetle's battle armor is fried by May Parker activating the jet engine, the appearance and arrangement of the building in the background changes between shots.
Factual error: When Susan is sorting Ruth's things, she finds Gordon's iPhone wrapped in a newspaper from 1994 with her voice-mail to him and realizes he went back in time, where Ruth killed him and took the phone from his corpse. Even if the phone had been turned off, after 16 years idle, the battery would have died long ago, but we see the phone is on with her message waiting. (00:55:50 - 00:56:45)
Revealing mistake: When Grapefruit cannonballs into the pull, it splashes on Pear. But no sign of any wetness is seen at all on him or the diving board.
Continuity mistake: When Mindy arrives at work, she goes into Danny's office and finds him on the floor in his underwear. She closes the door behind her, but a few seconds later, the door is wide open again. (00:02:25)
Factual error: Thomas holds the Gendarmerie rank of Capitaine (by which he is frequently addressed by his subordinates), and thus should wear three horizontal silver bars on his epaulettes. However, in the only scene where his rank is displayed he instead wears the two chevrons of an ordinary gendarme.
Revealing mistake: In the scene at the beginning when Scarlett is signing autographs after hers and Gunnar's small concert, you can see her signing a poster with a marker that doesn't leave any ink on the poster. (00:08:15)

Revealing mistake: Shortly after Kiera drives the van past the train tracks, we see a few different shots of Travis and Carlos fighting. In a couple of quick shots, Travis' stunt double can be seen. (00:13:00)
Visible crew/equipment: When Rebecca Madsen jumps in her car to pursue the armored truck, as she does a U-turn and takes off at high speed, you can see all the people standing on the side watching the filming. Some even have their phone cameras out videoing it. (00:29:30)

Continuity mistake: When Jacqueline raises a hand in response to her name being called, she puts up her left hand. A shot later, she has her right hand up instead.
Continuity mistake: Near the end of the episode, four contestants are called down: Jesse, Clint, Tatu Baby, and Steve. While the judges are explaining that three of them did not impress, a shot shows Sarah Miller standing to Clint's right, but she wasn't supposed to be there. Continuity mistake: In the Bond skit at the start, the cup on top of Rachel's head that supposedly has Scotch in it. When Donnie walks up to her and picks it up off her head to drink, the cup is empty. (00:00:50)

Factual error: Malloy has been missing all day and is starving from not eating. Yet his cheeks are sunk in like he hasn't ate for weeks. Your face doesn't get like that after not eating for a day.
Factual error: Dario Brando dies in 1880 (date on the gravestone) but when he was screaming at his son from his bed, Dio was reading "The Rise of Silas Lapham", which while being appropriate for his character, would be published only 5 years laters (started serialization in 1884).
