Continuity mistake: When Jack speaks to Michael and Jin, as they build the raft, and then when he walks up the beach and inquires about Sawyer's trouble with his head, the backpack that Jack carries on his back is nylon, black/green with white trim. Yet, when Sawyer asks, "Sensitivity to light, that bad?" the backpack is suddenly transformed into solid black leather, of an entirely different style. Continuity mistake: In season 1 it's revealed that Lynette and Tom first met when working for the same company, and Tom left his girlfriend for Lynette. In season 7 it's stated that they were already dating in college, and Tom betrayed Lynette with her friend Renee.
Continuity mistake: As House describes the lady's symptoms, in the front shot Chase has a small book/pamphlet in his hand. The camera changes to a side shot in the middle of House's description and suddenly the book is on the table and no longer in Chase's hand. This happens right in the middle of House's line and Chase doesn't make any quick movements to account for this discrepancy.
On The Edge - S2-E9
Continuity mistake: When Al goes to the ship to get the explosive, his dingy is powered by a Seagull outboard. When he returns, its a Mariner.
Rising (1) - S1-E1
Continuity mistake: When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate.
Continuity mistake: When Cassie is in the headmaster's office, her red pencil case keeps changing position from being held up clutched to her chest with her folder, to being in her lap.
Continuity mistake: When Erik enters his father's office the clock on the wall shows 5.10pm. After they have been talking for a couple of minutes the clock still shows 5.10pm.
Continuity mistake: When the video of Kimmy plays on the school announcements, and she talks about Duncan being hung up on Veronica, there is a cut to Duncan reacting with raised eyebrows in the foreground, while Veronica can be seen in the background with a passive expression. Then it cuts to a close-up of Veronica having a visible reaction with her whole face falling. But since Veronica and Duncan were reacting to the same thing, Veronica's reaction should have been visible in the first shot as well.
The Script and the Sherpa - S1-E5
Continuity mistake: When Vince and Eric are smoking weed with the older guy in the car, the older gentleman, Scott, is smoking from a joint less than an inch long, and proceeds to pass it to Eric. Eric, in turn, takes a hit and hands it to Vince, only the joint is now closer to two inches long.
Continuity mistake: When Dennis Leary and Michal J. Fox are at the bar, the shot Michael J. Fox has changes from being empty to partially full between shots, multiple times.
Continuity mistake: In Episode "Happily Never After": In the opening when the CSI team is looking at the puncture wound of Fiona Chisolm, there is no blood coming from the wound. It is actually mentioned that there is no blood. Then it is determined that she was dead for about 6 hours, but now there is an L-shaped blood drip pattern to the chest wound.
Continuity mistake: In the opening scene where Tommy and Jude are composing a song, Jude's head is turned to the side in a wide shot. In the following close-up shot, her head is staring straight up and not turned to the side.
Continuity mistake: When Brian and Gerry are chasing Kelly, Kelly's jacket is only zipped up at the bottom at the start of the chase, but when the camera angle changes it is zipped all the way up for the rest of the chase.
Kicking And Screaming - S1-E10
Continuity mistake: When Nikki is riding her bicycle while Cameron walks alongside her, a lock of hair is in front of her shoulder in a long shot, but all of her hair is completely behind her back in the close shots that immediately precede and follow the long shot.
Continuity mistake: Although this episode takes place during a single day, Boyce and Mac's hairstyles change from scene to scene.
Continuity mistake: In two episodes aired consecutively on CBBC on April 7th 2004, The "DON'T WALK/WALK" sign moves from one side of the crossing to the other and changes colour. At the end of the episode where Bernie has a lucky watch, the sign is to the right of the main door to the apartment building and has orange text. At the start of the episode where Bernie finds a cat, the sign is to the left, and red.
Continuity mistake: When Shirley walks through the door you can see over Alan's shoulder that he's reading the comics in the daily paper. As they talk the paper changes to the business section.
Continuity mistake: During the scene when Mickey walks into the urinals trying to steal the videotape, he has the phone in his hand calling the switchboard. He then puts it into his hand and turns the taps on. He washes his hands and leaves the urinals without the phone in his hand. Then, in the next shot, he walks through the door with the phone in his hand and puts it into his pocket. (00:11:00)
Continuity mistake: When Top tackles the villain into the water his black jacket has a huge NTSC logo on the back. The show cuts to commercial and when returns the villain and Tom fight in the water however the huge NTSC logo is gone from Tom's jacket. They wrestle under water and the villain floats face down to the surface revealing the back of a black jacket. The show attempts to add tension by tricking the viewer into not knowing if it's Tom or the villain's black jacket. (00:37:05)

Continuity mistake: Bette and Tina are with their sex therapist, and when she sits down her glasses are in her hand, from the back view they are on, and when it switches back to the front view, they are off again. (00:17:00)
