Visible crew/equipment: Beth attempts to convince Norman to inject himself with a tranquilizer by trapping him in a room and filling it with water. As the water rises, Norman frantically looks for a way out. There is a quick overhead shot of this scene that flashes on for just a second, and if you look around the perimeter of the room - you will see the entire crew, complete with camera and lights.
Suggested correction: This is simply not true. I have just watched that scene and didn't notice it, so I slowed it down to a third speed and still didn't see it.
Visible crew/equipment: In the scene where Michael is brought to Louis' house by his friends in the car, the camera rails are visible.
Visible crew/equipment: A boom mic drops in when the hitwoman is asking where Riley is, after the shootout.

Visible crew/equipment: About half the way through when Paul Newman is sitting on a seat in the police station with James Garner, the boom operator and another guy scratching his head can be seen to the left of Newman in the marble wall. It is fairly obvious taking up a 1/3rd of the screen. Also in the same time when Newman is walking through the halls of the police station with Stockard Channing a whole heap of movie posters can be seen on the walls like they used the offices of a movie studio as a police station.
Visible crew/equipment: Near the end, when the main characters lift the top of the well and shine the flashlight down inside, you can see the reflection of a stage-light in the water.
Visible crew/equipment: When Will Smith and Gene Hackman are fleeing the fortress, Will shoots at the pursuing truck with the shotgun, causing it to flip. As it lands on its top, the tube used to launch the pole that flipped it is visible.
Visible crew/equipment: When Marion is dead, you can see the camera panning back reflected in her eyes.
