Visible crew/equipment: When Henry helps pull Mark onto the treehouse at the beginning of the film, if you look very closely to the left you can just make out a crew member dressed in black helping him on to the platform. The crew member then ducks back under the platform.
Continuity mistake: When Tim and Lex first arrive, Lex is wearing a purple tank top with a colorful design, and in the following scenes or even between consecutive shots her tank top changes to completely different designs, though it's the same style of tank top.
Other mistake: In the scene where Madonna is pouring candle wax on Defoe, once they start having sex (with Madonna on top), you can clearly see Defoe's manhood poking out where it shouldn't. I realise they're not actually having sex, but shouldn't it be taped up out of sight?
Audio problem: Just before the crashed jet skids to a stop, you can hear the jet engines despite them being ripped off with the tail fin quite a way back, so you shouldn't be able to hear them.
Continuity mistake: Kimble dyes his hair very dark to escape detection and dyed hair fades over time. For the rest of the movie his hair noticeably goes from darker shades to lighter shades and back again showing that the scenes were shot non-sequentially on different days with a fair amount of time in between.
Plot hole: The opaque red lined map that Otomo retrieves from his first fight is ridiculous. Why would people who know where the hidden base is be carrying around a map on them (which shows a route so basic that they'd have to be brain-dead not to be able to remember it anyway) just so that conveniently one of their enemies can get it?
Continuity mistake: We see Sarah Jessica Parker well-dressed in red without a bra. When Bruce Willis remove her red robe, she's wearing a bra.
Factual error: When Jean-Claude is chased by the helicopter in the woods alongside the river, the man in the helicopter fires a MP5 machinegun, but it causes huge explosions, this is impossible, it's not a grenade launcher!
Factual error: When Cruz is schooling Jaunito about "Quetzalcoatl" in the background the song "Low Rider" is playing. The timeline states it's 1972, but the song wasn't released until 1975. (00:08:10)
Visible crew/equipment: In the scene where Tom Cruise is crossing over to Mud Island, you can see the reflection of the rotor blades on the monorail from the helicopter which is filming.

Continuity mistake: In the scene where Clarence is cleaning Alabama's wounds outside the airport (after she gets beaten up by and subsequently kills her would-be hitman), a large gaping cut seen on the left side of her face in one scene is gone in the next and does not reappear.
Continuity mistake: When Darian hits Cliff over the back with the attic pole, the brass hook disappears between strikes.
Continuity mistake: There's a scene where Oldman is talking to his wife (Anabella Sciorra) in the kitchen. He is sitting at the table on which there's a candle. In one scene the candle is lit, in the next it has gone out. It also switched between flickering and staying still.
Factual error: Throughout the movie, you see Carlito using a Beretta 92F semi-automatic pistol. The movie is set in 1975, that model pistol wasn't introduced until the mid 80s.
Continuity mistake: When Smith and Connor arrive at the Nakamoto tower and are exiting the elevator on the 46 th floor, you can see in several shots that Smith's suit is wet on the shoulders. However as he was going up the elevator the suit wasn't wet at all and even during the whole scene in the 46th floor, the suit changes from wet to dry as angles change.
Continuity mistake: The bullets that Leary removes from the keychain at the banquet are in a different position than they were when he put them in.
Suggested correction: We see him put the bullets in the rabbit's foot at least a day before the assassination attempt. Plenty of time for him to open the keychain simply to check and make sure he is fully prepared. Which does fall in line with his meticulous nature.
Continuity mistake: In one scene, Mark Hamill jumps into the passenger side of a car while running from men with guns; the men shoot at him, and the passenger side window is shattered by the gunfire. A moment later, once the car has gotten away, the passenger side window is intact.
Continuity mistake: After 70 days in the Andes, Nando has not grown a beard. Two days later, when they come with the choppers, he's managed to grow quite a nice one.
Suggested correction: It's possible that Nando continued shaving (offscreen) while they were living at the plane, but gave it up (or left his razor behind) during the trek out of the mountains. In real life, some of the survivors did shave while in the mountains.
