Plot hole: Detective Mason Storm has been in a coma for a number of years, so how is it that on the day he awakens from the coma, he not only has the strength to move his bed down corridors, but also knows the layout of the hospital?
Factual error: Asystole (the lack of cardiac activity, also known as the 'flatline') is not treated with defibrillation (electric shocks) but with alternating doses of the drugs epinephrine and atropine.
Continuity mistake: At the end, when Jack shoots the main bad guy, he had already double-tapped somebody, but then shoots at least 5 more shots from a 6-shot revolver.
Continuity mistake: When Harry Maddox is about to go for a ride with Dolly Harshaw, he reaches for his shirt and is mesmerized by the legs of his other love interest, Gloria Harper. Gloria catches Harry admiring her, with her head turned backwards, cigarette poised near her lips. In the frames immediately preceding this moment, she clearly does not have a cigarette, lit or unlit, anywhere near her.
Continuity mistake: When the nurse is wrapping a tensor bandage (also known as an Ace bandage) around Kinderman's hand and wrist, the way the bandage is wrapped changes between shots.
Other mistake: When Big Boys' men drive through the garage door trying to escape, Dick Tracy, the cops and the goons start opening fire. Yet, nobody seems to feel the need to stop and reload their weapon. They just keep firing without ever running out of bullets.
Continuity mistake: As the guys sit down for a beer and start talking, the viewing angle is from behind the one listening. As they switch to the other one who's about to say something the two bottles of beer magically turn around so you can always read the front label, no matter from which side the scene is shot.
Continuity mistake: About 20 minutes into the film Gene Hackman and his witness drives a big 4x4 through the mountains of British Colombia, trying to evade being shot at by gunmen in a helicopter. While they are being shot at, the windshield gets smashed, and is in pieces. The following scenes show the windshield intact one minute and smashed the next.
Continuity mistake: When Lily breaks into Roy's apartment, she unscrews two small light bulbs in the fixture next to the front door, throwing that whole end of the hallway into darkness. When Roy returns home, he opens the door and the hallway is brightly lit again. (01:32:45 - 01:34:15)
Revealing mistake: When Graham and Stella are in bed, the camera shows only their arms intertwined as if he's lying on top of her. She's gasping loudly in response to his thrusting, but their arms never move back and forth to reflect this, so it's clear they're not really having sex.
Continuity mistake: When Ralph is running from the hunters at the end and he finds the pig's head on the stick, he stops with his arms spread out a bit. In the next shot, even though you only see his head and shoulders, you can tell that his arms are now straight at his sides.
Deliberate mistake: It would be impossible for anyone to push up on a casket lid six feet underground and make the lid move much less crack enough to get their whole body through, the dirt would fill the box and trap the occupant.
Continuity mistake: In the strip club, the serial killer puts the dollar bill in the dancer's g-string twice.
Continuity mistake: The cabin is burnt. Costner comes back to the site to recover his hidden cash stash. Curtains blow in the breeze. Must've been woven from asbestos fibers.
Continuity mistake: When Baldwin kills the guy in the barber shop, he uses a Desert Eagle, a semi automatic pistol, yet it never ejects any shells.
Continuity mistake: In the car chase, the right headlight gets knocked out in the dark sedan, but later shots of the chase show both headlights working on all three cars.
Factual error: If you look at the band playing in the Regal Club the night the boy's mother first visits, the night where they first bring on the donkey, you'll notice that the drum kit used by the drummer in the band is, from what we can see, a 1980's Pearl kit, far too modern. (00:39:00)
