Factual error: When Rose is introduced to the entire team McGloin shakes her hand and says that he (respects or admires) anyone that has survived the POW school. She smiles, nods and walks away. McGloin then explains to the last remaining teammate that all NIOs attend the POW school and explains briefly what it is. First: It is the SERE school. Survival, Evasion, Resistance and Escape not really POW. Second: Not all, not even most, NIOs attend SERE school, just some.

Audio problem: When Muhammad is meeting with the group of Al-Quaeda leaders in the desert, the movements of his mouth do not match what he is saying in the foreign language.
Neogenic Nightmare Chapter 11: Tablet of Time - S2-E9
Factual error: When Peter is walking Alissa home, the traffic signal at the intersection they stop at goes from red to yellow to green. Not what US traffic signals do - they go straight from red to green.
Suggested correction: Not entirely true; although they are rare and may no longer exist now, I've seen traffic lights that followed the red, yellow, green pattern as recently as the mid-1990s.
Interesting. What state (s) did you see this in? I'm assuming going from red to yellow was to encourage cars to cautiously enter the intersection in case someone was running a red light?
Traffic lights in the UK do this - it's more to give you a second to get ready, in gear, etc., then as soon as the lights turn green you can go. Otherwise you get no warning of when the lights are about to change.
In Illinois; as I said, such traffic lights are rare, but they did exist at least as recently as the time this episode of the series aired, and they may still possibly exist in larger cities such as New York City.
This traffic light set-up (red to yellow to green) still exists today in the UK. From what I understand, it is to alert the driver that the light will be turning green imminently and to prepare themselves to put their car in gear, as manual cars are still pretty common in Europe. I'd wager this light cycle was phased out of North America due to the abundance of automatic cars today. Could have been different in 1994 though.
It should be noted that traffic lights that go from red to yellow before going green keep the red light illuminated so that both red and yellow are lit up. However, that's not what happens in the scene. I've never seen a traffic light operate the way it's shown. And Massachusetts still has traffic lights that go from red to yellow, however, when red and yellow are lit up together, this allows for pedestrian crossing.
Continuity mistake: Towards the end of the motorbike scene, Matt performs a wheely and overtakes Echo. In the following shot Echo is in front of him again.
Visible crew/equipment: In the rec hall, when Kensi feigns sickness while posing as a cadet the gunman jumps off the stage, and as Kensi bends over the small rectangular bulge of her mic pack can be seen at her back. (00:30:25)

Factual error: Scenes were obviously not filmed in London, more like New England. The vehicle registrations are the right colour, but wrong format.
Wanted Mans - S1-E5
Continuity mistake: We see the MI5 agents report Sam and Phil's identities to the Director. With the ID cards on screen, they state Sam as being 29, with a date of birth 25/03/1983. The series was released in 2013, so this age fits with the show's production dates of late-2012 or early-2013. However, Phil's age is described as 31 (but DOB is visible as 11/09/1985 - which would make him 26 or 27). Phil's age is detailed as 31 elsewhere in the series. (00:01:25)
Episode #1.1 - S1-E1
Factual error: This is set in 1933. When the killer comes back to his lodgings in episode 1, Lily on the stairs is humming "Somewhere over the rainbow", which was written for The Wizard of Oz in 1939. She also happens to wear bright red shoes, which feels like yet another Wizard of Oz reference. Interesting enough, the subtitles say that she is humming a different song, "Night and day" which she openly sings throughout the series and was already a hit in 1932. (00:44:10)
Continuity mistake: Right before Wendy sends the email that her husband is dead, a can of shaving cream can be seen on the right side of the sink. After hearing her husband come into the hotel, she knocks over the can of shaving cream, which is now on the left side. (00:47:05)

Factual error: There is a picture of a container ship with the caption "Lat 27.09 N Lon 5.2W" These coordinates are in the Sahara desert. (00:13:55)
Revealing mistake: When Harrow and Nicholls are in what is supposed to be Queensland Police Headquarters, a window is visible with logos for Radio National and Triple J behind them. This shows that the scene was recorded at the offices of the Australian Broadcasting Corporation. (00:31:04)

Continuity mistake: The establishing shot of the Agency's headquarters building zooms in on 3 tall, narrow windows recessed into a brick wall. We then cut to an interior view - of several wide, continuous windows without any recesses. They don't match the exterior windows at all. (00:07:30)
Factual error: Jonah tells Richard that Saturn has a "rogue moon" in a retrograde orbit that travels "at four times the speed" of Saturn's other moons. The graphic on Jonah's computer shows that the moon in question is Phoebe, which does indeed orbit in the opposite direction from other Saturnian moons. And the graphic correctly shows Phoebe's orbit as being much further from the planet than the other moons. However, as a direct consequence of its greater orbital radius, Phoebe is actually among Saturn's *slowest* moons. It has a longer orbital period (or "month") and a slower orbital velocity than all the Saturnian moons with smaller orbital radii, of which there are twenty.
Episode II - The Samurai Called Jack - S1-E2
Revealing mistake: When Jack cuts the flying car's hood off, he slashes it vertically, but it is cut horizontally.
Continuity mistake: In the episode "The Education of Jaime Sommers", when Jaime and Tom are having coffee, the size and position of cappucino foam on Jaime's nose changes from shot to shot.
Factual error: Molly and Ramsey find out that the van they are looking for was ticketed in Baltimore City at "the corner of Pennington and Fifth," and they are looking at a computerized map of the area. However, the map can be easily matched up to the actual Mapquest map of the area around the corner of Fifth St. and Patapsco Ave., except that the latter has been relabeled as Pennington. There is a Pennington Ave. in Baltimore City, but it runs parallel to Fifth, about a mile east of it.
