Continuity mistake: When Emma gets a pen out of her bag so that Dexter can write his number, the pen is upright. When the shot cuts, it is upside down, then it is upright again in the shot after that. (00:35:04)
Continuity mistake: When Grace gives Nadine a drink she takes the cover off and puts in on the table right in front of her. At one point during the scene it moves to the middle of the table.
Episode #1.1 - S1-E1
Continuity mistake: Episode 1 - When the commissioner kisses his wife at the beginning of the first episode, he is holding the holstered gun in his left hand and his ID in the right hand. That's in the first shot, because his daughter enters the room bringing coffee, and now the two items swapped hand. (00:05:10)
Revealing mistake: In the scene at the beginning when Scarlett is signing autographs after hers and Gunnar's small concert, you can see her signing a poster with a marker that doesn't leave any ink on the poster. (00:08:15)
Continuity mistake: After Melissa and Rhonda get off the phone, the way Rhonda is holding her phone changes between shots, and the phone screen changes from on to off.
Factual error: LtCol Baisley has his name and U.S. Army patches on the wrong side of his uniform. He has the Army patch on his right side instead of on the left. (00:33:00)
Episode #1.1 - S1-E1
Factual error: There is a view of the night sky in southern Africa. However, both the Big and Little Dippers (along with Polaris) are visible. In general, these constellations are not visible in the southern hemisphere, and where they are (North of 1° South latitude) they are very close to the horizon. The Big Dipper (Ursa Major) is to the left of the screen and Polaris (part of the Little Dipper = Ursa Minor) in near the top-center. This occurs shortly after the lion cub scene. (00:44:00)
Continuity mistake: At the diner when Tsunoda and Retsuko are talking, Tsunoda pulls out a red phone with a white heart on it from her purse. In the close up she is pulling it out with her left hand and the phone is horizontal with the heart on its side. It then cuts to a wide angle and suddenly she's holding the phone upright with both hands.
Continuity mistake: When Larry Bird hands Marty the pie, Marty takes it with one hand and then grabs it with his other hand. In the next shot, the pie is no longer in his hands and he's seen grabbing it with both hands at once.
Visible crew/equipment: Camera equipment is visible in the glass of the plane as the shot pulls in on the cockpit. It is fast but noticeable.

Audio problem: Kodachi grabs weapons from her locker and jumps in the night to go deal with Akane. She laughs maniacally but her mouth is shut. (00:07:55)
Episode #5.7 - S5-E7
Other mistake: Two actors are standing on an ancient muddy track which shows the tyre prints of a modern vehicle, made probably by the 4WD camera car. (00:09:00 - 00:10:00)
Character mistake: In Season 5, Episode 4 ("The Heart Attack Boys"), Abishola twice gives Christina an injection without swabbing her skin with alcohol. Abishola is an RN and would never make that mistake.
Continuity mistake: Before the concert when Carrie says she loves "purse wine" Anthony is looking at Miranda, but a shot later his head is turned the opposite way towards Carrie.
Other mistake: There is a countdown on the movie screen. It is then reflected in Dusty's glasses. But the numbers are not reversed in the glasses. (00:41:00)
Character mistake: S44/E80 Noah Newman works as a bartender, and while at Victor Newman's house says "let's show off some of those drink serving skills." While making a drink he pours straight vodka from the bottle into a martini glass and puts in olives and hands it to Abby to drink. As bartender Noah should know how to make a martini. He doesn't use any vermouth with the vodka and doesn't chill it first by shaking or stirring it before pouring it into the martini glass. (00:11:00)
Continuity mistake: As Dave is leaning over the edge of the air boat with Rose behind him, he pulls a chunk of metal out of the water and places it on the edge of the boat on his left side. When he gets up from his lying position moments later, the metal is gone. Moments later again, with an overhead shot of the boat, we see what appears to be that piece of metal positioned at what would have been his right side, not his left. This is also where we see him step over the seat (twice!) in subsequent camera angles. (00:30:00)
