Plot hole: In the heat of killing fake Red John, Jane forgot to notice that Craig O'Laughlin never knew about the mall setup to lure Red John. So definitely Gale Bertram himself was in contact with the serial killer which he may have missed first up, but all that time in jail afterwards he could have figured it out.

Continuity mistake: In S03E10 Patrick Jane obtains Laroche's suspect list "through back channels" from Virgil Minelli. In this episode Patrick Jane obtains Laroche's suspect list again, through bluffing with what Laroche is hiding in his safe.
Factual error: When Jane is changing the tire on his Citroen DS 20 he uses a car-jack, but because of that car's hydropneumatic suspension you don't need one. (00:00:19)
Suggested correction: Good one! The DS 20 is a fantastic car and you CAN use the high-lift suspension to lift the car up to change the tire, but you still need the jack stand - supplied in the Citroen kit - to keep the tires off the ground for changing. Most DS owners find it much easier to just use a regular jack.
Factual error: Lisbon is sending Van Pelt to El Oro State Forest. Van Pelt says, "El Oro State Forest, that's east of Lake Tahoe, right?" Lisbon says, "Yeah." East of Lake Tahoe is Nevada, not California. (00:01:17)
Continuity mistake: When Cho finds Archie's notebook under the bed, he turns to near the end and starts reading Archie's entry. But the position of where Cho has the notebook open changes, depending on the camera angle.

Continuity mistake: Cho opens the vent with a screwdriver and hands it back to the landlord (who pockets it). In the next shot Cho has it in his hand again finding the hidden drugs. (00:10:00)
Continuity mistake: Jane ordered the leather couch he is later seen napping on for the office in a previous season when he made a deal to be able to get a couch (only upper-level officers were allowed to have couches). Since this episode was a flashback to the first time Jane met and worked with the team, the couch he falls asleep on would not have been there, as that event had not yet happened.

Character mistake: Towards the end of the episode Patrick Jane plays a game of chess with himself. He moves the queen and states check mate. It is not checkmate, black knight would take the queen in the next move. It is in fact a flawed example of scholar's mate, something that someone of his supposed intellect and chess skills would not be rehearsing.
Continuity mistake: Lisbon has just returned from giving a presentation at the local school. She sits down at her desk, picks up two sheets of paper, and holds them with both hands. In the next shot, she's only holding one sheet with just her right hand. (00:01:25)

Character mistake: When Kim is trying to convince Lisbon to help the FBI on their case, she says Jane stopped the ten-year reign of a mass murderer. Red John was a serial killer, which, by the FBI's definition, is a criminal behavior distinctly different from mass murder; he may have had a large number of victims, but he killed them either one or a couple at a time, with a cooling-off period in between, not all at once.
Revealing mistake: At the very beginning, there's a square piece of board on the dead body's back, on which to support the knife.

Continuity mistake: In the meeting they said Erica Fline broke out of jail a year ago, but that occurred three years previously. (00:01:53)

Revealing mistake: Right after Jane and Lisbon rumble Erica Flynn and send video of her being arrested back to the FBI, Cho enters and asks to speak with Vega. In the shot of Vega sitting down, one of the computers in the office (centre of frame) has an Apple logo on the back that the production team forgot to cover up with a sticker like all the others.
Character mistake: During the Texas Hold 'Em poker game, George Holiday says, "What do we have on 4th street here?" But there is no 4th street in Hold 'Em. That's a term for a stud variant game when the 4th card is dealt.
Continuity mistake: Patrick Jane takes a sip of water just as he is about to propose to Lisbon. The way he holds the water bottle changes.
