Continuity mistake: After the car falls over the cliff, Roy and Johnny notice Dixie lying on the ground and in her closeup her sweater sleeve is rolled up above her elbow, but in the following shots that sleeve is rolled down properly until Roy pulls it up to take her BP.

Continuity mistake: At the very start, as the camera moves through Station 10, the Battalion Chief's vehicle is parked beside the fire pole, but when the tones drop and everyone rushes to the rigs, the Chief's car is now parked much further up, closer to the apparatus bay door.
Continuity mistake: At the scene of the traffic accident, after removing the injured driver from the overturned tanker, when Roy gets the Biophone from the squad and John runs over to the ambulance stretcher, notice the red rectangular material adhered to the front of Engine 51's cab. We know that Engine 51 does not have a license plate on the front of its cab, and during this scene Engine 51 is being played by the real Engine 127, whose license plate is at the front of its cab.
Continuity mistake: When Roy brings the injured tanker driver into the ER, the attendants wheel the stretcher down the hallway passing treatment 3, and after Roy asks Brackett and Early a pointed question about the trucker's condition, the doctors also walk down the hallway passing treatment 3, in the direction of the nurses' station. But then we see that Early and Brackett are with the patient in treatment 3, which is right beside the ER's receiving entrance.
Continuity mistake: When Station 51 is being dispatched to the brush fire, there's black tape on the chrome trim of the squad's lightbar, but when Squad 51 arrives on scene the black tape has vanished. Then later at Rampart, after Brackett calls Roy a hose jockey, the black tape is back, but vanishes again when they rescue the injured looter.

Continuity mistake: While Early is trying to remove a young boy's hand from a vase, in the shot from inside the room he's in treatment 3 with a windowless door, but when he steps into the hallway and asks Sharon for ice cubes he's at treatment 5 with a window on the door.
Continuity mistake: When the babysitter runs into Rampart with the child who swallowed phenobarbital pills, Brackett picks the boy up and carries him into treatment 5 (window on door), but in the following shots from inside the room we can see they're actually in treatment 2 (windowless door and set up differently). Then after they place the pacemaker Dixie leaves the room, and in the shot from the hallway she's coming out of treatment 5.
Continuity mistake: While Brackett is treating the man who was in a fight, it cuts to Early walking down the hallway, and in the shot from the hallway he's entering treatment 4 which is beside the nurses' station, but in the next shot from inside the room we see he's actually in treatment 2, the one beside the receiving entrance.
Continuity mistake: After Sharon calls out Dr. Early's name in the hallway, because paging him didn't occur to her, when she finds Early they walk into treatment 2 in the shot from the hallway, but in the shot from inside the room they're all in treatment 5, not 2.
Continuity mistake: When Roy breaks the key in the squad's ignition, while he's holding the broken piece in the two wide shots there's something dangling from the key's hole (another key/keychain), but in the closeup it's just the broken bit in his hand.
Continuity mistake: While Johnny and Roy are standing near the nurses' station, Johnny admits that he promised to take care of Paula's dog, and when Johnny turns his head toward Dixie and Brackett we can see the base station behind him, and in the next shot when Dixie and Brackett walk over, a clipboard has suddenly appeared between the telephone and Biocom EKG unit.
Continuity mistake: After Paula's taken in the ambulance, when Roy heads to the wrecked car and pushes the driver's backrest forward revealing Bonnie in the back, we can see that the passenger side door is closed, but in the next shot the passenger side door is wide open.
Continuity mistake: When Johnny's on the Biophone establishing contact with Rampart, there's a closeup of the GE telephone at Rampart's base station, and behind it we can see the tape machine switch is turned to the right, in the 'on' position, but when it cuts to Dixie walking over and responding, the tape machine is not on yet.
Continuity mistake: On scene at Paula's car accident, her houndstooth blazer is lying over the backrest of the front passenger seat, but when Captain Hammer opens the passenger side door it vanishes, then it continues to reappear, disappear, and change position in following shots.
Continuity mistake: When 127 hoses down the underside of the overturned trailer, we should be able to see the water spray while John and Roy try to get to the trapped driver in the cab, but we don't even though we hear the audio of the hosing.
Continuity mistake: While Squad 51 leaves the brushfire area with the looter (Tony Dow), when Johnny requests a water drop the helicopter pilot says that he's Copter 2, but the footage we see of the chopper flying overhead is of Copter 14 and Copter 10.

Continuity mistake: When the Battalion Chief's car arrives on scene at the factory fire, he and the driver get out of the car, then it cuts to shots as they both stand beside the wall while the Chief talks to dispatch over the HT, but a few shots later we see the driver just getting out of the car as Engine 36 passes.

Continuity mistake: While Roy shows John the squad's right, front compartment, in the closeup of the Datascope a clipboard/notebook with a stack of papers suddenly appears atop the OB kit, which vanishes again in the OB kit's closeup.
Continuity mistake: After Johnny calls Dixie to inquire about Paula, when Johnny heads to the kitchen and asks which one of the guys wants to take Bonnie home for a couple of days, everything on the table keeps changing depending on the camera angles.
Continuity mistake: After the guys played cards, Johnny's low so he gets to do the dishes, and when Johnny proposes dealing another hand to see who gets the dog, cards appear/change on the table between shots.

Correct, it was J. Pat O'Malley...he also played the grandfather with his grandson when their rocket exploded, and also played "Old Bill" in the episode with Ann Prentiss, where Gage saves the little girl from the burning tree house, and her mother falls in love with him.
