'V' For Vashon: The Patriarch (3) - S5-E11
Character mistake: When Ben talks to the old guy about Spider Brown, Ben says, "Somebody that got rich the past week." The time between the shooting and trial was probably months, not just one week. Spider would have been flashing money right after the hit, and then disappeared.
Character mistake: McGarrett mispronounces his old friend Cargill's name in the bank vault scene, giving it a soft G and calling him "Car-jill." Earlier in the episode, and in all the scenes after this, he correctly says "Cargill" with a hard G. (00:17:00)
Not That Much Different - S1-E23

'V' For Vashon: The Father (2) - S5-E10
Character mistake: Chin and McGarrett call the bomber Marcos, but his name on the social security card says Macros and Vashon calls him Macros.
The Capsule Kidnapping - S8-E22

The Ninety-Second War (2) - S4-E17
Continuity mistake: When Vogler stands in the doorway talking to Danno, the position of his arms alternates with each change of shot, from down at his sides to propped at 90-degree angles on his hips. (00:42:05)
Answer: He was fired. He never really appreciated his character. He felt Kono was portrayed as a big dumb Hawaiian and that the stereotype was racist. He also felt underutilized. He was fired after a heated argument with the show's publicist regarding his character. It seems there's not much details given regarding the incident or the firing, so it's seems possible he upset the show's producers as well.
