Character mistake: The General picks up the comms microphone and starts wrongly speaking into the back of it. (00:01:00 - 00:01:55)
Fatal Cargo - S4-E7
Revealing mistake: The seam on the back of the gorilla costume is visible as he attacks the doctor's campsite.
Continuity mistake: Kowalski's uniform and hair are soaked after sealing the hatch from the hull breach, but his uniform is bone dry and hair properly combed when he immediately reports to the Seaview's Control Room.
Factual error: Captain Crane's periscope view of the boat accident survivor is an overhead and down view, while a real periscope view would be level with the ocean surface.
Visible crew/equipment: The boom mic's shadow is visible behind Riley as he speaks with Dr. Holden.
Continuity mistake: Part of Admiral Nelson's torn uniform changes position on his shoulder as he is being checked out in sick bay.
Visible crew/equipment: The Seaview is attacked by an enemy sub. During the torpedo assault, wires used to rock the Seaview model are visible just above the submarine's aft bow. (00:23:15)
Other mistake: Aside from the error of having mines so densely packed and so close to the bottom so deep, there is no possible way the Seaview could have settled to the bottom while passing through this minefield without being destroyed. (00:48:00)
Other mistake: One of the natives putting the captured Seaview personnel into the cave is wearing laced up sneakers. (00:36:25)
Continuity mistake: Captain Crane removes a failsafe key, which is hanging outside his uniform collar, from around his neck. It wasn't there in the previous shot. (00:10:30)
The Return of the Phantom - S2-E26
Deliberate mistake: Kroeger's ghost takes over Crane's body when the captain is injured, but still alive. In "The Phantom Strikes," Kroeger told Nelson his host had to be dead first. This "fact" needed to be altered in the sequel so that Crane could survive to be rescued at the end. (00:16:00)
Visible crew/equipment: When Nelson says, "Let's get out of here," the shadow of the entire camera rig moves by on the side of the navigation console. (00:07:45)
No Way Out - S1-E12
Continuity mistake: When Victor tries to get into sickbay, the guard stops him, pointing a rifle at his chest. But in the very next reverse angle shot, the rifle is again in its original position in the guard's hands, pointing at the ceiling. (00:35:10)
