The Night of the Murderous Spring - S1-E27

Continuity mistake: In the scene after the opening credits, West's hair is brushed back from his right side. It stays that way for the rest of the scene - as he is shaving, after he is drugged, as he is shooting at Dr. Loveless, as he looks out the window at the fallen Loveless, and as he runs out of hotel looking for Loveless. While West is still outside, Gordon arrives, and West's hair looks as if has gel in it when he is talking with Gordon. After the first freeze frame, West's hair goes back to the non-gel look.

The Night of the Murderous Spring - S1-E27

Continuity mistake: When Dr. Loveless falls from the sill of the double window, the right half of the frame is open (the camera view being from the outside looking in). When West rushes outside to find the "body" missing, the right frame has closed itself and the left is now open. (00:08:40)

The Night of the Bubbling Death - S3-E1

Revealing mistake: As West drags the body of a bad guy out of the scene, the supposedly unconscious man is holding his head up so as not to have his head dragged through the dirt. A truly unconscious person's head would flop back on to the ground.

The Night of the Infernal Machine - S2-E14

Trivia: West collides with the cook and sends him sprawling face-down in a huge platter of gelatin. Before racing off again, West quips, "Sorry about that, chef!" This was a winking homage to the 60s spy comedy Get Smart and its bumbling hero Maxwell Smart's famous catch-phrase, "Sorry about that, Chief!" (00:47:30)
