Revealing mistake: After Pope enters Scofield's cell to see the escape hole, we see him standing outside the cell looking out at everyone throwing toilet tissue etc. If you watch closely right after we see a guy pulling his pillows toward him to tear up, it cuts to another inmate who is throwing stuff out as well. Only thing is, we see the cell door slides to the side slightly which would indicate that the cell doors where not locked. In the event of an escape, lockdown would happen immediately, with all cell doors locked with the prisoners inside their cells. (00:04:00)

Plot hole: When Scofield comes back after visiting his wife, Fernando announces his arrival. However the problem is, everyone is inside digging the hole. As Fernando walks in along with Scofield, it leaves no one outside watching for guards. The guys are at least smart enough to keep the watch outside instead of all staying inside. If a guard shows up unnoticed, they are screwed. (00:09:40)
Factual error: Lincoln Burrows is scheduled to die on May 11th, 2006. When Veronica learns of the priest's death, we see a wall planner on the wall with May 11th circled in red marker. The day on the wall planner is Wednesday. However, May 11th, 2006 was a Thursday. (00:32:00)
Plot hole: C-Note says about pulling a simple bump and grab on The Key like they did before. However, when Abruzzi and his men get The Key the 1st time, C-Note wasn't in the group then. It is extremely unlikely they would divulge this trivial piece of information as the issue is long since resolved. (00:03:50)
Continuity mistake: When John and Michael meet with Philly, he has his hands at his sides. Camera changes and they are on the table, clasped. (00:20:40)

Factual error: D.B. Cooper made his escape on November 24th 1971 with 200k in $20 bills. Westmoreland shows Michael a $100 bill saying it's from the ransom. Impossible. (00:36:00)

Continuity mistake: When Michael is finishing up the Allen key, he is wearing his grey sweatshirt. Yet when he finishes it and checks it on his tattoo he is wearing his blue shirt over his sweatshirt. (00:36:00)
Factual error: Scofield has two toes cut off. However, his recovery is so quick that it's just plain stupid. He would be limping badly for at least a month.
Riots, Drills and the Devil: Part 1 - S1-E6
Character mistake: Before the riot breaks out in Sick Bay, Dr. Tancredi calls the black inmate "Kwame." The inmate in question is credited simply as "Stroker." "Kwame" is in fact the actor's real first name - Kwame Amoaku. (00:24:15)
Continuity mistake: After the gas canisters are thrown into the rabble, Michael runs back up into his cell and puts his hands on the bunk, they are clean. The shot cuts to T-bag yelling up at him and when the shot goes back to Michael, his hands are covered in blood. (00:32:30)
Suggested correction: When Michael stumbles into his cell and leans against the wall, there are blood trails over his hands. He then stumbles against the bed, and his hands are out of shot but his wrists are still visible. Then, when he falls back against the wall, his hands are visible with the blood trails.
Factual error: They have made it a point that Scofield is incarcerated in a prison in Illinois, and even gets married so he can be eligible for conjugal visits during his stay so he can pass information or items to his "wife" or she to him. In Illinois, conjugal visits aren't allowed. Only something like six states allow them, and this isn't one of them. Totally a plot hole, and since this thing was so carefully planned out, I would think he would run across this information ahead of time.
Riots, Drills and the Devil: Part 1 - S1-E6
Factual error: During the last several minutes when the Governor and warden are discussing the current situation and the status of the governor's daughter, a cut away shot is shown stating that it is the Capitol Building Springfield, Illinois. The Capitol building shown is in fact the Capitol of Connecticut, in Hartford CT. (00:31:40)
Factual error: When they have escaped, and the dogs are barking at them as they hide in the bushes, this couldn't have happened the way the show presented it. The spot that they escaped is on the main road (Collins Street, also, it was to the left of the screen from where Joliet Jake came out in 'The Blue's Brothers) The dogs are barking at them as they hide in the bushes, and that location is on the -other- side of the Prison. There is NO way that they would be able to circle around the prison and get from one side to the other considering in the front, there are fences surrounding the main building, obstructing you from going that way. The back side of the Prison (There it a road next to a junk yard located right next to the Prison, the same road that is used to get to the 'Gate' the cons are looking at when the dogs are barking. That road would have been full of cops, since all the cops are in the parking lot. It would have been more believable if they would have went across the street to where the women's facility was, and were shown on camera looking across to the parking lot out front. (00:05:50)
Continuity mistake: When Abruzzi's lawyer tells Abruzzi he is broke, Abruzzi points at him saying "I told you from the beginning what would happen if he screwed me." When it cuts Abruzzi's hand is suddenly lowered. (00:15:00)

Continuity mistake: In the episode "The Old Head" when Michael gives the cat back to Westmoreland, from the side, the cats paw is on his shoulder but when the angle changes to the front, it has moved down to his chest. (00:17:45)
