Continuity mistake: When Starbuck returns in the season 3 finale, the side of her Viper has no nameplate on it. However, during the season 4 premiere, the nameplate with "Kara Thrace 'Starbuck'" has returned, though only a moment later.

Continuity mistake: In the season 3 finale, Anders has a very noticeable limp, especially seen when he's following the Chief to the hangar deck, However, in the season 4 premiere, even though the same day, he has no limp at all.
Revealing mistake: Just after Tigh announces that Centurions will be "pouring in at platoon strength", the next shot shows those Centurions marching though the Galactica's halls. None of them are casting shadows despite the various nearby light sources that should be causing such shadows. This is most obvious looking at their feet.
Other mistake: In the intro, when Tigh, Tory and Tyrol meet, Tigh is wearing Admiral's rank insignia of his uniform. His correct rank is Colonel.
Plot hole: During the last Galactica jump, Kara enters different coordinates that she puts together in her head from her memories. There is no explanation as to how the fleet came up with those coordinates instead of the rendezvous coordinates. If the fleet had them as the rendezvous coordinates, Kara wouldn't have said "I don't have the rendezvous coordinates!" Right before the Galactica jump. Bill Adama also wouldn't have said in response "It doesn't matter! Just jump is out of here..." (00:05:54)
Suggested correction: In the flashback, Kara said "I thought that, if I assigned numbers to the notes." Those were the numbers she used for the notes to "All Along the Watchtower." She remembered them and used those for the coordinates.
Continuity mistake: In the medical area, the Doctor checks on Samuel while others look on. Sharon has both her arms down by her sides. The angle changes, just as he says "lot of visitors", and Sharon's suddenly clasping her hands together.
Continuity mistake: Near the end of the 4th season episode "The Oath", Adama and Tigh choose to stay and fight rather than flee Galactica with Roslin. Apollo is not shown voicing any opposition to this. Yet in the next episode's opening, he is shown confronting his father, saying "No, Dad, you can't stay here". While this moment can be excused as having happened off-camera as the mutineers were shown approaching in the prior episode, it is the later show's narration which states "Previously on Battlestar Galactica" that is technically incorrect.
