Continuity mistake: In the shot of the 6th Precinct police station, it has a California state flag flying out front. The episode took place in Massachusetts, at the Massachusetts Institute of Technology. (00:07:45)
Plot hole: Scully's statement that she was 23 when she wrote her senior thesis in college throws off her timeline of college, med school, residency, FBI Academy, Quantico teaching assignment for two years, and X-Files assignment at 28. Her undergraduate degree in Physics should have been completed when she was 20 or 21 to allow at least 4 or 5 years for her medical training. (00:41:35)
Deliberate mistake: When the unconscious, naked Asian doctor is removed from the warming tub, his hands are clasped over his crotch to protect his modesty, despite the fact that he shouldn't be able to do that, since he isn't conscious and his previously frozen limbs are now pliable and relaxed due to the heat.
Continuity mistake: We see Scully wearing pants while sitting on the couch, but when the real Mulder comes in, she jumps up and we see she's wearing a skirt. (00:38:35 - 00:41:25)
Factual error: The date stamp says that it is April 12 and Scully says that it is Sunday, but April 12, 1997 was a Saturday. (00:03:10)
Visible crew/equipment: When Scully asks Fox "Do you know what day it is?" the boom mic drops down at the top of the screen. (00:04:50)

Continuity mistake: During and just after the Mulder's seizure, when Scully's face is shown, it is at the level of a standing person which suggests that both agents are standing, but in the next shot Mulder and Scully are crouching. (00:31:35)
Continuity mistake: When the CSM discovers the bright spot on the carpet, he immediately realises that Mulder's apartment was under surveillance. However, one can notice that soon after entering Mulder's apartment, the CSM looks down at the floor and carpet covered with blood and there is no bright spot. (00:14:00)
Continuity mistake: During the meeting at a horse track, after the CSM says "You need my expertise, " the First Elder raises his head. In the following shot from the other side, he raises it again. (00:21:00)

Continuity mistake: Although the subtitle states 12:00 AM, Mulder's watch hands mark 02:35. (00:43:16 - 00:43:39)
Factual error: When Byers asks Langly to try under active cases to find Holly Modeski in the FBI database, Langly only presses two or three keys on a keyboard, which is obviously not enough to enter "Holly Modesky" to the search engine. (00:19:40)
Factual error: When two unidentified men who approached Susanne Modeski in the warehouse take out their handguns, they have regular pistols. However, when they start to shoot, continuous fire is heard that could only be produced by an automatic weapon. (00:28:40)
Suggested correction: Fully automatic pistols exist, such as the Glock 18.
Continuity mistake: When the Lone Gunmen speak with Susanne Modeski, in the background there are some cars parked in front of the Stadium Inn. However, just seconds after, when the telephone rings, there is a white truck parked at the entrance. Later on, the cars are back again. (00:38:55)
Visible crew/equipment: When the Lone Gunmen catch up with Suzanne at The End of the episode, when she is talking to them and the camera is on the three, you can see not one, but two boom mics at the top of the screen. (00:40:20)
Factual error: When Suzanne Modeski retreats to the restroom with a pair of pliers to pull out her tooth, she is only gone for approximately 15 seconds before the gunmen follow her. For a petite woman to rip out a strong, healthy molar from her own mouth (which also had metal prongs Underneath in her gum) would not take so little time, no matter how determined she was. It's also highly unlikely that the lack of anaesthetic would have allowed her to do so without making pained noises.
