Continuity mistake: At the beginning of the episode, a bunch of money falls from a building. Of course people are grabbing it but when the First Responders show up, everything has miraculously been cleaned up. No money to be found. With the amount that has fallen and the chaos, there would likely be some left over. The street is squeaky clean.

Continuity mistake: When the father of the kid in the yellow convertible is on the phone with Maddie, in the second and third shot he can be seen holding his iPhone upside down. Cut to the fourth shot of him on the phone and his phone is magically in the right direction. (00:02:53)

Continuity mistake: When the father arrives at his son's car crash, there is a low shot looking up, specifically at the father. The sun is almost overhead. Cut to the rest of the action, the sun is further down in the sky.
Continuity mistake: At the end, Hen is in the rage room. She puts on a helmet, but in the next shot, she is smashing some glass without wearing the helmet. (00:41:26)

Continuity mistake: When all the firefighters and EMS teams are coordinating in the big dirt parking lot with the view of downtown LA behind them, the sun changes several times. Its either later in the day or earlier, depending on the angle. Look at the scenes with Athena talking to another character. The sun is in front of them. Cut, and the sun is overhead.
Continuity mistake: In the first segment it says the baby was born in 2011. Then later in the show the baby is not even one, but the year says it is 2015. So the child should have been at least 3-4. Not an infant still. (00:00:11 - 00:13:16)
Continuity mistake: At the beginning of the episode, the oil spray pattern cleans itself from the firefighters' face shields in the span of a few cuts.
