The Two Against Time Raid - S1-E24
Revealing mistake: When Sgt. Troy fires his submachine-gun at the German guards the barrel of his weapon is pointed well above the Germans, yet they still fall as if being hit by the bullets.
Other mistake: The shot of the German soldier firing the halftrack's machinegun is repeated in a later scene, due to the cover of the ammo loading tray popping open the same way each time.

Continuity mistake: Hitch and Troy park their jeep six feet from the rear of the half-track, but the next shot shows the half-track parked just inches away.
Revealing mistake: The scene of the German tank is obviously reversed as American tanks (the ones painted dark yellow to pass for German tanks in the show) have the commander's cupola on the right side of the turret. Also, a swastika on the rear of the tank is reversed.
