Continuity mistake: When Danny and Baez confront Carmine to clear a few things up, Carmine tells them to call his lawyer. Just as Carmine clicks his key fob and his car explodes, Carmine's coat is entirely unbuttoned, but when he's blown backwards and slams onto the ground, in the first overhead shot of Carmine lying on the ground, his coat is buttoned up. Continuity mistake: Danny and the ESU enter the warehouse basement to find Bobby, and when they start to return fire, the first guy they shoot knocks over the white plastic lawn chair (beside the recliner) as he goes down. Then, in the next shot facing the door, that white lawn chair is upright (not the yellow table chairs), but in the next shot it's lying on the floor again. (00:32:40)

Continuity mistake: While Danny and Waylon Gates are with Rodriguez at the safe house, showing him the photograph of his daughters, during this scene there is no picture hanging on the wall beside the door, but as they lead Rodriguez out in the last shot of the scene, a picture suddenly appears on that wall. Continuity mistake: While Danny and Baez are questioning Damian about Mason's whereabouts, Damian's right handcuff switches to the opposite way depending on the angle of the shots. Either the single metal bar faces up or the double bar faces up, which is quite impossible to change when it's locked around his wrist.

Continuity mistake: When Erin is in the ladies' room walking toward the door, she finds Anthony "lurking" in the hall, and Erin's eyeglasses which are hanging from her crew-neck top go from facing her right side, to her left side, then back to her right side, between shots. (00:31:55)

Continuity mistake: During the stakeout of the apartment building, in the exterior shots of the Clyde Motorcycles shop, Danny is peeking out through a ripped paper opening within the letter "D" in Clyde. However, in the interior shots the black tape letters of the word Clyde differs. The paper is ripped between the two letters "D" and "E" and that space within the "D" is blocked with molding, making what is in the exterior shots quite impossible. (00:15:40)

Continuity mistake: As the couple with their two children are walking down the street, the daughter, Sophie, is holding latex helium balloons which are the transparent type, but after she and her mother are struck by the reckless drunk driver, when the balloons float away they are the opaque type. Continuity mistake: During the family dinner, while everyone discusses the use of body cameras Frank is wearing his eyeglasses, but in the shots facing Linda during the conversation Frank's eyeglasses are gone. (00:21:40)

Continuity mistake: In the first two shots of the family dinner Henry is holding his fork up in his left hand, but in the third shot from behind him (facing the family) his raised left hand is empty with his fork lying across the plate, then it's back up in his hand in the fourth shot. (00:21:10)

Continuity mistake: When Frank goes to see Gallo at the hospital after the lawyer's arraignment, Frank drops a deck of cards on the bedside table beside the bottle of whiskey, but the bottle has vanished in the next shot. (00:41:00)

Continuity mistake: At the start of the episode, when Frank opens/closes the refrigerator door we see the door's layout, and the door bin cover is under the top shelf compartment. As he makes himself the sandwich Erin and Nicky walk in, and when Nicky opens the fridge door it's an entirely different interior style with the door bin cover at the top.

Continuity mistake: During the courtroom hostage scenes, when Raoul Delgado and Erin are on the phone with Danny there are some closeups of the gun Delgado is holding. After Delgado hangs up on Danny, when he slams the gun into Jack's temple and orders Erin to go with him, in these closeups the gun is noticeably different. (00:30:55)

Continuity mistake: After Danny shoots Delgado, he kneels down and holds Erin at her right side with his back facing the car (driver's door is open), but in the next shot Danny is kneeling at Erin's left side with his front now facing the car, as seen in the live feed Frank is watching on the monitor. The live feed is not backwards, note the open driver's door, etc. (00:34:35)
