Continuity mistake: When JP gets his hand bitten by the rat, after he throws it away from him, there's a close-up of his hand covered in blood. In every shot before and after, there's little to no visible blood on his hand.

Visible crew/equipment: When JP notices the box has been taken out of the statue and hears a noise coming from inside, there's a dramatic shot where the camera swoops from one side of the statue to the other, and you can see the shadow of the cameraman pass over the statue, plain as day. Continuity mistake: When Joey and Terri are talking and the puzzle-box in the statue gets brought up, in the reflection on the window, you can see Joey begin rubbing her neck with her right hand. When it cuts to the opposite angle, her hand is no longer up by her neck.

Continuity mistake: When Joey gives Terri her cigarettes, when Terri asks, "You gonna have one?", her hands go from on to off the ashtray between cuts.

Continuity mistake: When Terri and Joey are talking about how Terri doesn't dream, throughout the scene she's carrying a cup of coffee. But at a certain point in the scene, her coffee cup just completely vanishes. There's no point where she sets it down. It's just there... and then it isn't. It's entirely possible she set it down in a deleted shot, but the way the scene is edited, it's all portrayed as one fluid conversation with no pauses, so it's a mistake.

Continuity mistake: When Terri is talking about how she doesn't dream, she walks around Joey's living room. The scene cuts to a different angle, and she flops down into the chair. However, before the cut, she wasn't close enough to the chair to sit down. Additionally, Joey's hands change positions between cuts.

Continuity mistake: When Terri goes to Joey's apartment and talks about how she doesn't have dreams, as she stands up, her hands go from far apart to close together between cuts.

Audio problem: When Joey is awakened from her dream about her father's death, she picks up her phone, but the sound effect of her phone being pulled off the base doesn't quite match. (It plays the sound of the receiver fully being picked up before it's fully picked up.)

Audio problem: When Joey is arguing with the editor about whether or not there's a "story out there," there's a shot where Joey firmly says, "THERE IS a story out there." But her lip movements show her saying, "There's a story out there" in a more laid-back tone.

Continuity mistake: When the guy slams his hand on the bus window, startling Joey, in the first shot, only his right hand is on the window. In the next shot, he's hitting the window with both hands. He then takes his hands off the window and starts lowering them, but in the next shot, his right hand is back on the bus.

Visible crew/equipment: After the clubgoer's head explodes, we see Joey riding in a bus. In the first shot, as the camera moves towards Joey, towards the end of the shot, you can just barely see the reflection of a crew member in the window. He appears to be pulling or steadying the camera rig (probably a dolly camera rig or something similar).

Continuity mistake: When Joey runs towards the operating room and Terri leaves the hospital, in the first shot, Terri begins to open the door and walk out in the background. The camera cuts, and we see her do the exact same thing a second time.

Audio problem: When Joey sees the clubgoer being brought into the hospital with Terri, as they hurry down the hall, none of the dialogue that is overdubbed matches anything being said. Especially the lines, "You ever seen anything like this?" and "Yeah, yeah! Sure." The camera lingers on the actors' faces as though they should be saying these words, but their lips don't match at all.

Audio problem: When Joey and Doc are introduced and Doc says, "Yeah, while, like you said, it's a mystery. But that's all it says: a mystery," his line is overdubbed. Several of the words don't match his lip movements.

Continuity mistake: When Joey is introduced and says, "That's a wrap," she starts rubbing her forehead with her hand. When it cuts to an over-the-shoulder shot, she's no longer rubbing her forehead. (You'd see her hand still from the angle.)

Trivia: The radio we see Joey find in the dream is actually the exact same radio seen in "Hellbound: Hellraiser II." Not the same make and model... the EXACT same radio. The production designer put an order in for a period-accurate, old-timey radio for the World War I scenes, and through sheer dumb luck, the prop house just so happened to send them the same radio from the previous film.

Trivia: Clive Barker originally had no involvement with the film. He was brought in during post-production to help fix the film, evidently quipping, "Alright, now let's make it into a 'Hellraiser' film!" after seeing a rough cut.

Trivia: During the sex scene with JP and Sandy, JP aggressively holding Sandy's breasts wasn't scripted or planned. The actress decided she didn't want to bare her breasts on camera, so she had the actor playing JP hold them whenever she was on camera.
