In the shot, we can see that the inmate's face has been covered in white powder to make him appear as if his face has gone pale due to the blood loss, but it makes his face look completely unnatural, and not at all how a human face would ever appear in a situation like that. Continuity mistake: When the assistant warden starts searching Ricky's cell, he opens the two doors to the toilet stall to see if he is in there. Whilst Ricky jumps down from the ceiling, the assistant warden has the left door on the toilet stall held open and the right door on the toilet stall closed. When he notices Ricky in the next shot, the left door on the toilet stall is now closed and the right door on the toilet stall is now wide open. (01:19:09)

Revealing mistake: Just after Ricky has killed the bald inmate, he then goes to punch the assistant warden in his face, resulting in his one real eye falling out. In the close-up shot of the assistant warden getting punched, a fake prop dummy can be seen instead of the real assistant warden, most likely used so that it would be easier to create the practical effect of his eye falling out. After the warden asks Ricky a question and Wong Chun stops hitting him for a moment, we can see that Ricky's left cheek looks fine (aside from the blood running down his head). Continuity mistake: When Ricky stands back up to start fighting with Wong Chun again, a slash wound can be seen on his cheek, positioned beneath his nose. In the shot of him bursting through the door, we can see that the interior of the door is extremely thin and is barely a millimeter deep, revealing that it is a fake prop door and not steel.
