Continuity mistake: The damaged headlight of the car in which the kid and mom are running at the start switches sides. In the first shot when the car breaks through the door it's the left headlight that's not working, but on the road it's the right headlight that's not working. (00:03:30)
Continuity mistake: At the start the black SUV clips a red car and we see the headlight break. A few shots later the SUV is fine. (00:03:35)
Continuity mistake: Moreau puts the picture of the boy on the bike. When Johnny picks the picture up its position has changed. (00:08:20)
Continuity mistake: When Johnny is talking to Vasil, the phone in Vasil's mouth changes position. Notice the little bit missing from the side of the phone. (00:32:20)
Continuity mistake: When Nadya escapes with her son from the monastery she drives the car through the doors, and the driver's side of the car sustains substantial damage at the side of the hood with the side-view mirror completely broken off, but during the chase there's little damage to the car's body and the driver's side-view mirror is now dangling at the side.
Continuity mistake: During Nadya's escape from the monastery, there are exterior shots of Nadya through the windshield and there's nothing hanging from the rear-view mirror, but in the interior shot (viewed from the back seat) just as she swerves away from the red car, there's a small cross on a cord which is hanging from the rear-view mirror.
Answer: The entire film was designed to have a different tone and feel from the first film, to be accessible to viewers who never saw the original. Thus this film exists as something of a soft reboot. In interviews the directors also stated the origin story for Ghost Rider never made sense to them and they wished to change aspects of the character for this film. That includes removing the Penance Stare and making Ghost Rider have an uncontrollable desire to feed on the wrong people have committed, even things like little white lies could set him off.
