Continuity mistake: In the scene towards the end of the movie when Julia is in the mental hospital, it shows her getting out of bed and trying the door, which is locked. The handle to the door is on the left side. But if you go back and watch the alternate ending and she goes to try the door, the handle is on the right side of the door.

Entered into a pychiatric hospital, Julia looks through a window when suddenly "They" break through. We don't see what happens. Cut to Dr. Booth and an intern coming into the room. The Dr. and the intern look in the closet for Julia but see nothing. In reality, Julia is there in some sort of black ink world surrounded by "They". The Dr. and Intern leave closing the closet door and the world is dark. All of a sudden "They" jump on Julia and the movie ends abruptly. It isn't clear if Julia was killed, tortured, eaten, etc. It also raises a theory that "they" aren't real and she is imagining everything.
