Continuity mistake: During the title sequence two villains are playing an implausible-looking chess game which includes two black bishops both positioned on black squares. Black takes white's queen. A moment later we see the player putting the taken queen aside but numerous pieces have moved, including two knights and several pawns. (00:03:05)
Factual error: In the opening scene, Bond throws a stun grenade into the room with the guards. It hits the chess board. But the chess board is on a table in the middle of the room. There is no way Bond could throw it from upstairs, through the window and have it hit the top of the table in the middle of the room. Not even if the grenade could jump. (00:03:10)
Revealing mistake: At the beginning of the film, Bond is on a training exercise to free a trapped hostage. Since it is only a training exercise, everyone involved is using blanks in their weapons. Why is it that when Bond fires an automatic rifle, we hear multiple bullets ricocheting? (00:03:25)
Continuity mistake: When Bond enters the room to free the woman there's a chair next to the bed. A shot later it's a meter away. (00:03:40)
Revealing mistake: After the opening scene, M is viewing it all on tape. But the tape shows the scene in the same way as the audience saw it - the line of sight is changing, zooming in etc., much more than hidden surveillance cameras could accomplish. (00:04:20)
Continuity mistake: When Bond arrives at Shrublands the butler passes by the columns towards the car. Shot changes, and he is meters behind walking towards the columns again. (00:06:35)
Factual error: When Lippe bumps into the shelves with glasses they are all upright. It is impossible for one of the glass pipes to go straight into his back. It would require the glass was lying flat on the shelf. (00:22:25)
Continuity mistake: When the nuclear missiles land in the ocean, the man on the boat guides them down. On the screen in front of him, the range goes from 12 to 4 km away, but then to 5 km and 3 km away. (00:30:05)
Audio problem: When the capture of warheads is announced by Blofeld, the Foreign Secretary tells M that he will have to reinstate the 00s. M says, "As you wish," but his mouth doesn't move. The scene was dubbed. (00:34:05)
Factual error: Nigel Small-Fawcett introduces himself to Bond as being from the "British Embassy, Nassau." The Bahamas is a British Commonwealth country, and the British diplomatic mission in Nassau at the time of the film was therefore the High Commission. A British diplomat would not have referred to it as an embassy. The High Commission in Nassau closed in 2005. (00:44:05)
Factual error: When we first see Fatima Blush, she is waterskiing. She is doing several stunts, which are only possible with one ski, the most notable being skiing backwards with one foot in the air. However, when she releases the tow rope and skis up the ramp into James Bond's arms, she is on two skis. (00:45:55)
Continuity mistake: When Bond meets Fatima he says he wants to fish. Her hair is behind her shoulders in the wide angle, but over her shoulders in the close-up. (00:47:14)
Continuity mistake: When the shark is pursuing Bond through the sunken ship, you see first Bond, then the shark coming through the door. The bubbles generated by his movement abruptly disappear as the shark comes through, revealing too obviously the stitching together of the two sequences. (00:50:50)
Revealing mistake: There is an underwater scene where Bond is swimming in a sunken ship, trying to escape a shark. At one point, he swims into a cabin,and the sharks head comes through the door. The camera then goes to a close up of Bond's hand trying to latch the door, while the sharks head is pushing on it. The film is then reversed several times to make it look like Bond is trying to push the door shut while the shark is pushing it open. (00:51:40)
Continuity mistake: When Bond is on the phone to Mr Smallfoot he is making love to a fishing lady. She is on top but next shot he is. (00:55:45)
Suggested correction: Before Smallfoot calls Bond is actually lying on his left side and leaning over the lady, facing the window. To answer the Smallfoot's call, he rolls over onto his right side, facing the phone. He then rolls back over onto the left side and leans over the lady again. While it does appear that the shot from before the call may have been reused for this, there would be plenty of time for him roll over from one side to the other, which only takes a moment.
Continuity mistake: During the massage, Bond and Domino talk about Mr. Largo. Behind them is a woman who stands up and leaves, leaving the place empty. Shot changes and the place is suddenly packed with women. (01:02:07)
Continuity mistake: When Bond and Largo are playing Domination - in the round about The World - Largo is in pain when viewing his face close up, but in a short clip, where he can be seen through the playing screen, he is smiling. (01:16:05)
Audio problem: In the tango-scene the music doesn't match the moves. And since Bond is an excellent dancer, he wouldn't make such a mistake. (01:17:30)
Audio problem: When Bond and Domino are dancing the tango, we hear Domino ask, "What is it you're after?", but her lips don't move. (01:17:40)
Audio problem: When Bond shoots Fatima with the pen, first the is a small puff-sound, and then a larger boom-sound after the projectile hits her, but before it explodes. What makes the second sound? (01:28:35)
Suggested correction: The pen rocket didn't use an impact detonator, so the miniature rocket had to expend all its fuel before its explosive payload detonated. That was the weapon's malfunction when Bond tried to use it on Fatima Blush: the rocket did not expend all its fuel and simply lodged in her clothing. Then the rocket fuel reignited, producing a sputtering hiss for a moment before the powerful explosives actually detonated, blasting Fatima into confetti.
It's also a three-stage blast. The first bang is Fatima's torso detonating (which is actually not that large an explosion), followed by another blast that disperses fuel for the third blast, which was pretty powerful for an indoor explosion.
