Visible crew/equipment: When El Wray and the police arrive at the hospital as Dakota drives off, at the very back of the shot is the crane used for the lighting (confirmed on audio commentary).
Factual error: The movie was meant to be set in Oregon, but it was actually filmed in Shreveport, Louisiana. In the scene that has Tracy leaving the apartment complex where Bafford (Mr. Smith) is, she is worried that she is being chased by a jogger who may possibly be Mekes. In the background, during the shot where the jogger first appears, a brown sign can be seen. These specific signs are unique to Louisiana and describe the history of certain locations, this one obviously being Shreveport.
Continuity mistake: The writing on Ghost's sword early in the film has the Turisas rune (representing TH in Thunder) near the hilt, but later not only is a different font used but now the Dagalas rune (representing the D in sword) is at the hilt.
Revealing mistake: There are 4 different vehicles in the movie that have the same license plate number (2PCE275). The first is on a white car owned by the gang member that shoots Max. Then you see the same plate number on the pickup truck of the skinheads that are harrassing Gary. Then you see the same plate on Steven Seagal's car as he drives away from saving Gary from the skinheads (incidentally, the front plate on his car is different than the back plate). The last time you see the same plate is on a Lexus driven by the guy in the all white suit who is going to buy the cocaine.
Continuity mistake: Right after Jim has his nightmare while sleeping in the car, he looks around for Grace, who is standing outside the car looking at scenery. When Jim sees her, she has her arms crossed over her chest. The next shot, with Jim walking around the car, she has her arms by her side, and immediately after, the shot with Jim approaching her, she has her arms crossed again. All this took place in about 2 seconds, not enough time to drop her arms and recross them exactly the same way as the first shot. (00:19:35)
Character mistake: The endotracheal tube (breathing tube) was not secured to the patient. It is standard practice to secure this tube to the patient in order to keep it in place.
Other mistake: In the end credits, the character played by Tina Sloan is listed as "Stationary Saleswoman". The correct spelling of this character should be "Stationery Saleswoman", as she sells stationery. She doesn't sell while standing in one spot.
Revealing mistake: In the scene at the beginning of the film when the bus enters the village, you can see that the road signs are on the left side of the road (not facing the bus) and the continous line that separates the two ways is on the exit of the curve, not before the curve. This is because the action of the movie is set to happen in England but that scene was filmed in Romania, a country with right-hand traffic. (00:03:20)
Factual error: After the little girl is taken they play audio of the mother on the phone with 911, the operator says they can't do anything for 24 hours. Police do not wait 24 hours to look for a missing person, especially a child.
Continuity mistake: When being shot at by the angry waiter, the right hand door mirror was hit by the rifle shot. A few seconds later a view shows only a very minor hole in the glass, and no damage to the rest of the mirror. Surely a rifle shot would have caused more damage to plastic and chrome trim, almost surely passing right through. (00:30:20)
Continuity mistake: Near the beginning of the film when Abby and Neil sit down at the table drinking wine, the amount of wine in Abby's glass changes from almost full to almost empty between shots.
Visible crew/equipment: When the ambulance is about to crash into the tanker, right before and during the crash a camera mounted (painted white) on top of the cab is seen.
Continuity mistake: In the beginning scene at Vincent's trailer, Errol is looking through the cupboards as he questions Vincent. He comes across an adult magazine, and flips open to a page. He flips another page or two over, and the exact same picture is shown as the first picture he opened to initially.
Factual error: Contrary to popular belief, a person won't freeze immediately in outer space, as depicted in the movie. Space may be cold, but it's also a great insulator and the biggest problem for astronauts is dissipating the heat generated in their bodies.
Factual error: In the beginning when Anwar is in Cape Town, South Africa, according to the shadows cast by the men in Anwar's party, it's about 1 pm. He makes a call to Isabella in Chicago, United States when he apologizes for not calling her earlier. Cape Town is 8 hours ahead of Chicago, it would have been pitch dark in Chicago at that time but Isabella is outside in the midday sun playing soccer with Jeremy.
Factual error: When Tom shocks Angela with a stun gun, she gets knocked out. That's impossible. Stun guns don't knock you out. Stun guns don't cause unconsciousness.
Continuity mistake: The left tail light of Sadie's car is broken when she picks up the unfortunate hitchhiker. The light works when she turns left off the road. (00:36:20 - 00:38:35)
Factual error: Right after Walter has the nightmare about killing his wife, he wakes up and turns around to see the alarm clock show 11:12. The clock makes a flipping sound when it switches to the next minute, just like those alarm clocks from the 1980s that have those little numbered plastic cards inside. However, it's a digital alarm clock and they do not make such sounds.
Revealing mistake: Sean Patrick Flanery plays a nerd who has trapped a female vampire in a cage. About 3/4 of the way into the movie, he tricks a co-worker into visiting, planning to feed him to her. He extends his right arm through the bars, and she starts to caress and kiss it, then begins to bite it and suck his blood. When he tries to withdraw, she tears off his arm at the shoulder. But in the next shot, he and Flannery both hit the floor outside the cage, and the co-worker's right arm is visible. However, when the scene begins, both Flanery and the co-worker are wearing watches on their left wrists, and in the shot of them on the floor, both have watches on the right, suggesting that the image has been reversed.
Revealing mistake: When Billy is looking at Nick's driver's license, for sex it says F for female.
