Revealing mistake: As Reg fights with one of the baddies with the chainsaws, as the baddie take a swing with the chainsaw he misses and cuts into the wooden door. You can clearly see that a vertical cut has already been made in the wood though, to help the chainsaw get in and cut. (01:17:55)
Audio problem: In one scene where Jackson is caught by some bad guys you can see (and hear) that a couple of times when Jackson laughs it is badly dubbed over.
Visible crew/equipment: When Carol-Anne is running away from Kane's voice in the garage, she ends up standing in a puddle, only to have hands pull her in seconds later. But when the camera moves down Carol-Anne's body to the puddle, you can see the platform the actress is standing on.
Revealing mistake: In the last scene of the film where the Reverend is holding up the glass jar with the mini-blob in it, you can see the mechanism being used to move the blob (look between the Reverend's fingers). (01:30:30)
Continuity mistake: When Jackie, Tim and Luke are at the bar, the bartender places a glass of milk beside Jackie's right hand. When it cuts, it's between his hands. (00:36:15)
Factual error: Ivan Danko's police uniform is a combination of at least six different uniforms. His hat is a Moscow Commander's Special and he also wears a Parade Band Musician axelbant.
Continuity mistake: When the killer is using the R/C car bomb to follow Harry and his partner; as the R/C car approaches Harry's stopped car, it starts to swerve to the left, as if to go around. However, in the next shot, the R/C car is perfectly straight again, and lined up to go under Harry's car.
Factual error: The sequence showing the moon is incorrect as it shows the Moon as it looks from the northern hemisphere. It should be inverted.
Factual error: MacKenzie is referred to as "Gunnery Sergeant". This is not a rank used by British and Commonwealth forces. He actually wears the rank insignia of a Flight Sergeant.

Visible crew/equipment: When Mel Gibson is getting his red boat ready to put in the water, his friend comes out of his house to talk to him. You can see the camera crew reflected in his friend's sunglasses.
Plot hole: When Gary Busey infiltrates the Mexican terrorists' compounds, he first throws a stick to distract some guards and kills them. A few minutes later a terrorist officer reports to his boss that Gary Busey has killed three of his best men. Are we to believe that the three best terrorists were tricked by a stick thrown into some bushes, and then easily killed by an unarmed assailant (they had submachine guns)?
Visible crew/equipment: When the General is explaining to the scientist that everyone in the immediate area must be killed to contain the virus, the shadow of a crew member is seen on the wall behind the General.
Continuity mistake: When Beau Bridges runs past the hooker on the street he has no bow tie. In the next scene as he approaches the telephone booth, the bow tie is back and he removes it after entering the booth.
Audio problem: When Bourne rescues Marie, he fires a silenced pistol at the man who was trying to rape her, but the sound is of an unsilenced gun. (01:01:10)
Character mistake: When the little boy is reading his report about Abraham Lincoln, he states that Lincoln was born in a log cabin in Illinois. Lincoln was actually born in a log cabin in Kentucky.
Continuity mistake: After Chuck Norris throws away the pimp during the initial evacuation of the embassy, the man pulls out a gun and shoots Chuck Norris from behind. The bullet hits him in the right shoulder but at the next moment - seen from his front - the wound is on his left side.
Revealing mistake: When Jemma enters the headshrinker's laboratory, there is a table with three heads on it each encased in glass. When it goes in close-up on the middle head, you can see that the table has a hole where the actor can slip in his head.
Continuity mistake: The main villain is trying the soft approach and wants to persuade David's ex-wife to side with him. As they are having a conversation about the bank loan, John Steiner's glass is raised - or not - depending on the camera angle. (00:37:00)
