Visible crew/equipment: In the final scene before Phil Daniels rides the scooter off the cliff you can see a shadow of the helicopter filming the scene.
Visible crew/equipment: After Ripley fails to shut off the auto destruct sequence, she runs through a corridor filled with gas. As she exits from the gas right before the shot cuts away we see her pass by a full sized light. Beside this light is the clear arm and head of a lighting crew member. (This is acknowledged on the DVD commentary).
Visible crew/equipment: As the boat crew steps ashore to find Kilgore, the obvious shadows of two crew and their equipment are seen moving across the backs of the soldiers and then the raised netting slightly further away. This isn't the documentary crew they meet shortly afterwards, who are further onto the mainland. (00:27:20)
Suggested correction: In this scene, the boat crew immediately encounter a camera crew filming the soldiers. This is their shadows.
While they do encounter a documentary film crew, this does not happen until a few seconds after the mistake occurs. The shadows of the film crew are shown when they are walking onto shore, whilst the documentary film crew are shown more onto the main land. Plus, it seems a little weird Francis Ford Coppola would decide to randomly include shadows of a documentary film crew when the cameo doesn't happen until a few seconds later. Clearly this is the shadow of the camera and cameraman cast on them, and is a valid mistake.
Visible crew/equipment: Carnival biker chick goes through a flaming hoop and falls down after going though it. Just before she goes, you can see a rope tied to her to pull her off the bike after she passes through the flames. (00:33:30)
Visible crew/equipment: In the non-widescreen version, a hole in the ceiling and a microphone can be seen picking up the dialogue between Patrick McGoohan and Clint Eastwood in the warden's office.

Visible crew/equipment: In a scene where Kirk Douglas appears to 'leap' onto his horse, a crew members arm is clearly visible in the foot of the shot pushing a mini trampoline next to the horse, ready for Kirk to jump onto and bounce up onto the saddle.
Visible crew/equipment: In the film George is backing his Rolls Royce up at Hugh's place, as he's doing that the film crew's reflection can be seen on the car. (00:08:00)
Visible crew/equipment: When Wild Bill crash lands his P-40 Tomahawk in the street, then dazedly tries to "bail out", watch his parachute. A nearby explosion is supposed to catch the parachute and pull him backwards, but the 'chute doesn't react to the explosion. Instead, a line attached to the end of it pulls Wild Bill backwards.
