The Strike - S2-E4
Character mistake: The boy says he's on McConnell Street, but the street sign says it's McConnell Avenue. (00:16:03)
The Strike - S2-E4
Continuity mistake: When the boy opens the double doors to peek in, he only opens them wide enough that his eye is visible. In a following shot, they are now open wide enough that most of his face is visible. (00:12:01)
The Strike - S2-E4
Continuity mistake: When Mitch and the boy shake hands, they're on the very edge of the water - it's barely ankle deep, and there are a bunch of people behind them. When the shot of them being struck by lightning is shown, they're much further into the water (calf-deep), and the people behind them have disappeared.
The Strike - S2-E4
Continuity mistake: After Mitch and the boy are struck by lightning, two male lifeguards run towards them to save them. In subsequent shots, they change into another different male lifeguard and a female lifeguard.
The Strike - S2-E4
Continuity mistake: When the boy walks into the water, the same extra (guy in black shorts with a white horizontal stripe) runs past him in the background twice in quick succession.
The Strike - S2-E4
Visible crew/equipment: When Mitch is thrown off the building by the bald guy, the next shot shows it isn't Mitch falling - the hair is far too dark and the facial shape is completely different. (00:30:40)
Revealing mistake: Mitch is carrying a doll during the chase on foot – it's way too floppy to be an actual person.
Other mistake: The credits for the episode have simply been duplicated from a previous episode, "Takeover." The credits state that the guest stars are John Furey and Sandra Hess, neither of whom appeared in this episode. Both actors, in fact, appeared in the episode "Takeover." (00:41:56)
Plot hole: Garner is absolutely rubbish at playing basketball in this episode. The problem with this is that in an episode of Baywatch ("Lifeguards Can't Jump") - which happens in the same universe - his character is shown as being a very skilled basketball player.

Visible crew/equipment: During the montage of the cast getting ready, when Angie Harmon is posing in the mirror, you can see the cameraman reflected. (00:04:10)
Factual error: Donna jumps off the bridge (which is relatively low down), into shallow water. She then narrowly avoids being hit by a huge ocean liner. There's no way such a huge ship would be in such shallow water - for starters, it couldn't cross under the bridge, being far too big. (00:03:12)
Visible crew/equipment: When Donna jumps off the bridge, her falling body is a dummy/mannequin of some sort - its arms are stiffly by its side, and it has no face. (00:01:54)

Continuity mistake: When Donna jumps off the bridge, it's into relatively shallow water under the bridge, and there is land very nearby (at both ends of the bridge). When the camera shows her in the morning, she is suddenly far out to sea in the ocean. (00:01:51 - 00:09:33)
