Visible crew/equipment: When Frasier is standing through his sunroof to speak with the drivers stuck behind him in the parking garage, one shot shows Frasier's body and much of the rear window which features a very clear reflection of the boom mic.
Other mistake: When Sylvia is outside the Sheffield mansion she says "I wonder if my daughter Fran is at home? I will see if she is" the doorbell rings as soon as she finishes that line, but you can see both her hands are visible. She can't have rung the doorbell.
Factual error: A character in the episode has invented denim, but denim was invented in 1873 and was being used widely up to the time period of the series, which is set in 1893.
Continuity mistake: The age gap between Eric and Cory decreases as the series progresses. At the start of the show, Cory is in 6th grade and Eric is a high school sophomore, meaning they are four years apart, give or take a few months. By season 3, Cory is in 9th grade (confirmed by dialogue mentioning that Shawn, who is in the same grade as Cory, is 15 years old), and Eric is a senior, leading to an age gap of three years. The first episode of season 4 takes place a couple months after the end of season 3, with Eric and Cory returning from a summer Road Trip, but just two episodes later, Cory mentions that he is in 11th grade, while Eric is still barely out of high school, leading to an age gap of two years.
Revealing mistake: There are numerous times throughout the series where the supposed big city of Metropolis is actually just a back lot set and none of the buildings are more than a couple of storeys high. Quite often the green hills of Southern California can be seen beyond the set and between the buildings.

Continuity mistake: When Filburt first reads his fortune cookie, it says "Bad luck and misfortune will infest your pathetic soul for all eternity". Continuity mistake: Gary has to feel around in mid air for the portal as the wall has just been bombed. But when he reaches through, later, his arm comes out of the wall which has somehow returned.
Continuity mistake: When Yakko points to India, Africa, except for Egypt and Madagascar, is shown as one country until he points to Tunisia.
Continuity mistake: When Grace and Nadine are at the bar, Nadine starts to take money out of her purse. The shot changes and suddenly there is money on the table. Then the amount of money on the table keeps changing between shots.
Visible crew/equipment: After Max walks in with the dog, she tells Khadijah why Walter was kicked out of obedience class, and when it cuts to Tripp the end of the living room set wall is visible on the right side of the screen, so we can see the kitchen set. (00:10:40)
Used Car/Bounty Hunters - S8-E9
Continuity mistake: In "Used Car", when Beavis and Butt-head first approach the car, it's in between two trucks, a white one and a brown one. At the end, the spot where the car was is now between a green car and the white truck.
Suggested correction: The entire series was set in an alternate history of sorts, so most, if not all, historical "inaccuracies" cannot be regarded as mistakes.
