Visible crew/equipment: After Indy and Henry have escaped from Castle Brunwald, Indy jumps into one of the boats, pulls the motor starter cord and jumps back out, then just as he bends over to release the boat from the piling, right between Indy's legs the black covered arm of a hidden crewmember appears from under the tarp taking hold of the throttle, steering the boat away from the pier. Visible crew/equipment: When the Batmobile is driving itself to Batman and Vicky Vale, you can clearly see a hand turning the steering wheel. (01:12:00)
Visible crew/equipment: Right as Young Biff starts to chase after Marty through the tunnel, there is a shot of the wheel spinning around, and you can see the camera crew reflected in the hubcap. (01:29:45)
Visible crew/equipment: During Bud and Coffey's fight in the lab, we see a lightbulb swinging from side to side between the two men. If you look closely, you can see a hand (belonging to a third person) pushing the lightbulb. The only people in the room are Bud and Coffey. (01:44:16)
Visible crew/equipment: When Riggs first starts shadowing Rudd, he cuts off Rudd's car with his truck. They go to the close up shot of Riggs and when he drives away you can see in the reflection of the trucks window, the PANAV on the hood of the Panavision camera. (00:48:35)
Visible crew/equipment: In the elevator shaft/rocket boot sequence, the 'arm' of the lift device that is actually holding Spock casts a strong shadow on the wall to our right of Spock, indicating the arm goes from Spock's back to the black vertical slot in the right wall. It is most visible as Spock first lowers into view next to Kirk. (01:02:40)

Visible crew/equipment: When The Punisher is being tortured on the 'rack', look on the floor. You can see the tape on the floor for 'blocking' the actors. Some of them actually stand where they are supposed to.

Visible crew/equipment: Near the beginning of the movie when Nick is on his motorcycle going to get one of his kids to take to school, as he picks the other kid up to kiss, you can see the reflection of a circular lamp on his black helmet visor.
Visible crew/equipment: When the Gates family is chasing the bad guys in the cemetery and one of them enters the Snake Pit bus, you can see the actor's mark on the floor. (He does manage to hit it.)
Suggested correction: This would need more clarification. I couldn't see any string in the mentioned sequences - and for the record, I don't think strings were used, you can easily find interviews with the production team that mention the different trick at work here (they quite literally 'shot' the chairs up in the air).
