Revealing mistake: In a scene towards the end, we see a crowd of Vietnamese on a riverbank firing their machine guns into the trees...one soldier clearly walks directly in front of another soldier who continues firing away without hitting him.
Revealing mistake: When Bronson is chased up onto the rooftop by one of the gang members, Bronson pushes the guys over the edge of the building, and it is visible that it is a dummy taking the fall.
Revealing mistake: Remo, clinging to a tree trunk, is shot at by Grove using a vehicle mounted machine gun. Pieces of trees are seen shot but this is only sound and special effects. There are no muzzle flashes from the machine gun, the cocking handle does not move in time with the shooting, nor does the cartridge belt move through the weapon.
Revealing mistake: During the Army assault on Ortega's compound, Jackson is shown as he uses a machine gun to clear a long balcony of enemies. The third person he kills here is hit several times, but does not fall over the railing. Instead he slowly and carefully climbs over it, lifting one leg over at a time.

If you look real closely at the last mortar to hit the second American, it is a scare-crow type figure dressed in military fatigues that takes the mortar hit.
Revealing mistake: Just before Michael and Ellie get out of his car, a fire extinguisher is seen mounted above the dash. Michael opens the hood and the engine begins smoking. Michael reaches into the car for the extinguisher. You can see through the windshield that he is handed another extinguisher instead of removing the one from its mounted spot. The mounted extinguisher is still visible in its holder. Likely done to preserve pacing so he doesn't have to fumble with the clamps on the mounted one.
Revealing mistake: During the opening assault, notice that the sandbags of the enemy fortifications are more like fluffy pillows; they are squashed under a single person leaning on the bank, and they wobble and nearly fall over when Carrasco himself brushes the hip against them walking by.
