Continuity mistake: When Sam is looking at the mirrors with night vision, we see him pause and say to Dean "Look at this." The camera then cuts to an extreme closeup of the camcorder screen and the image on the screen is different to what was on there before. (00:18:15)

Continuity mistake: Adam swishes his hand in the filthy toilet, getting brown liquid over his hand. Adam's hand suddenly becomes completely clean a very short time later. (00:12:50)

Other mistake: When Larry is taking the cigarettes out of the box, look at his hand. It has a white paint drop or white dust on it, but throughout the scene it disappears and reappears. (00:55:10)

Continuity mistake: After Xavier kills Jonas with the spiked baseball bat, we see Amanda, Daniel, Addison and Laura in a hallway. Laura collapses to the ground due to the nerve gas that they've all been inhaling. Between shots her left arm moves from her stomach to across her right shoulder. (01:02:00)

Continuity mistake: When Tapp and Sing go to Jigsaw's place and find the guy strapped to the chair, they uncover him, then re-cover him to see what Jigsaw is going to do. When Jigsaw pulls the sheet off, one side is still caught on the drill bits. When he starts the drills the sheet is completely clear. (00:48:00)

Continuity mistake: The mark on Larry's face changes throughout the film. (00:12:30 - 01:08:45)

Revealing mistake: When Doug comes in at the start and says he has been riding Ritchie's hog, all the stage lights can be seen in Carrie's wine glass. (00:02:20)

Continuity mistake: When Adam throws the picture that he found in Lawrence's wallet to him, the picture lands next to the rectangular piece of tile that Lawrence uses in the immediate next shot to bring the picture closer to him. The tile moves from in front of him (given it's proximity to the photo) to right beside his right arm/hand. It "moved" about two feet. (00:59:10)

Continuity mistake: When Adam is walking into his apartment, his camera has a short lens on it. He then rounds the corner and goes into the darkroom and the camera has a long lens on it. (01:04:20)

Continuity mistake: When Adam retrieves the bag with the saws from the toilet, he places it on his right side. At one point you can see that there is nothing on his left side, but the next shot shows Adam grabbing the bag from his left side and tossing it into the tub. (00:14:00)

Revealing mistake: When Amanda is slowly getting up right after being thrown into the needle pit, if you watch very closely, in one or two quick shots you can see a small pad on the actress' back used to hold the needles in place. Slow motion helps, but is NOT required. (Seen in Uncut version, though it may be in the theatrical version as well.) (00:51:10)

Continuity mistake: When Dr. Gordon gets out of Tapp's car and is talking to him at the car window, the amount of shirt sleeve showing under his jacket changes between shots. (00:42:00)

Continuity mistake: When Amanda is thrown into the pit of needles, in some shots there is only one, while in others, there are two or more stuck in her back. Also, in some shots the needles are pointed at an angle, whereas in others, they are pointed almost straight up. (Uncut Edition) (00:51:20)

Continuity mistake: When Amanda and Daniel entered the bathroom from 'Saw', Adam's body is shown with his right leg shackled. However, in 'Saw' and in Adam's death flashback in 'Saw III', his left leg is shackled. (01:14:10)

Continuity mistake: Carrie and Arthur are talking about Carrie making a Thanksgiving meal, and as they are talking the frozen meals in Carrie's trolley change position between shots. (00:03:10)
