Continuity mistake: When Squad 51 is dispatched to 'man down', as the squad heads out of the apparatus bay, the engine we see parked in the bay is the old Crown from seasons 1 and 2, instead of their current Ward LaFrance.

Revealing mistake: At the house fire, after Roy and Johnny go upstairs to rescue the invalid woman, Captain Stanley and Chet head to the front door, and just before Cap kicks in the front door, we can see that the door isn't actually closed, it's just ajar.

Continuity mistake: At the decompression chamber on Catalina Island, after John and Roy introduce themselves to Dr. Scott, the doctor removes the stethoscope from around his neck and puts it down, but in his closeup it's still around his neck, then in the wide shot it's gone, only to return in closeup again.

Visible crew/equipment: After Brackett tells Donny's mother that her son does not have meningitis, it cuts to an exterior shot of Roy and John driving in the squad, and the moving shadow of the filming vehicle is visible on the ground, in front of the squad.

Continuity mistake: Dispatched to the structure fire are Station 51, Engine 36, Engine 116, and Truck 3. While Johnny and Roy are searching the rooms for the young girl, it cuts to a shot of guys from 9, and later there are also shots of 28's and 19's guys, even though none of their companies were called to this incident. These are shots from previous episodes including 3x7, "Promotion."

Continuity mistake: When Station 51 is dispatched to the truck fire, as Captain Stanley responds to the dispatcher Roy and Johnny get into the squad, but we see Marco, Mike and Chet head back to the kitchen instead of getting on the engine, then we see both the engine and squad responding to the incident.

Continuity mistake: When Ruthie is wheeled into the ER, Dixie tells the attendants to put her in the treatment room where Dr. Early's waiting, and they wheel her into treatment 2 which has a windowless door, but when they're in the room the door has a window, which means she's in treatment 1, not 2. Plus the fact that both treatment rooms are set up differently.

Other mistake: When the woman from the salon is brought into the treatment room she's unconscious, but as they transfer her from the ambulance stretcher to the hospital gurney, she lifts her left arm from under the stretcher bar and then also positions it on the gurney.

Revealing mistake: When Squad 51 leaves Rampart, in the first interior shot facing Roy as John says, "Yeah, I'll tell ya, when it hits ya. Boy when I met Valerie it hit me," through the back window we see movement at the back of the squad. Then in the shot facing Roy as John says, "So that's that... That's exactly how it is," through the back window we see a crew member's face (sideways), he has a mustache (John does not have a mustache), right behind John's head, as he's riding atop the back of the moving squad. Continuity mistake: When Deke comes into the ER with the two burned firemen and speaks to John and Roy, the window behind them is on the same side as the ER's receiving entrance. Notice that outside the window there's one of those blue metal shades at the top, which does not exist in the exterior shots of that side of the ER.

Visible crew/equipment: When Roy, John, and Vic are inside stage 3 trying to outrun Charlene, Roy starts climbing down the first ladder, and we can see equipment moving right beside Roy, then when Roy climbs back up and they find the second ladder, more movement can be seen at the bottom of the screen.

Continuity mistake: After Roy and Vic run out of Stage 3, when Johnny backs up and bends down, we can see Charlene, the bear, on all fours, then in the next shot the bear is walking on her hind legs toward the door, but in the next shot from behind Johnny the bear is still on all fours and further back again.

Other mistake: After Johnny takes Captain Stanley's BP, when Chet tells the guys, "Animal fat is the root of all evil," and also after John jokes, "How to survive on no meals a day," check out Roy's feet because he's wearing shoes (looks like black sneakers) that are below the ankle, with no socks. Then in a few moments they're all dispatched to the 'possible heart attack' and when they're at the scene, while Roy is sitting with the patient, he's wearing socks and his uniform shoes (their tops are above the ankle) as usual.

Character mistake: After Captain Stanley tells the guys about the diet and nutrition study, Cap tells Roy that he and John should go to Oceanside Park for a fire inspection, but later when Johnny's on the HT he tells dispatch to respond Engine 51 to Oceanland Park, not Oceanside Park, and then the dispatcher gives the incident address as Oceanland Park.

Continuity mistake: In the opening shot of Roy and Johnny at the whale tank, the maintenance guy behind them is standing between the red bleacher seats and purple seats below, but in Roy and Johnny's closeups the guy is standing between the two top purple bleacher seats, then when the whale splashes, it goes back and forth yet again.
