Other mistake: When Natalia is messaging Boris for the first time, the keys she is pressing are not what appears on screen. For example, watch closely and she hits 2 buttons at the same time which when you compare it to a keyboard is the letters "p" and ";" However, the ";" never appears on screen. (00:55:00)
Revealing mistake: During the tank chase, there is an overhead shot of the tank sliding round a corner. In this shot you can see scars in the road from a previous take. (01:19:40)
Factual error: In the cemetery, Bond encounters Alex Trevelyan, whom he thought had been killed in the chemical plant explosion. One of Treyelvan's henchmen shoots Bond in the right side of his neck with a dart but in the very next scene, and for the remainder of the movie, Bond's neck shows no sign of having been punctured. Revealing mistake: In the opening scene when Bond is shown running up to the spot from where he will bungee jump, it is not Pierce Brosnan but his stunt double. You can also see a smudge on the camera lens in the same shot. It starts out on the wall on the left and moves downward onto the asphalt and stops just to the left of where Bond drops the bungee cable. (00:01:05)
Revealing mistake: During the shot where Bond and Alec are emerging from the hallway floor in the dam compound, watch the left wall. As Bond bumps against it after pulling out his pistol, it visibly flexes inwards and pops back out. A concrete wall capable of supporting the weight of a dam with industrial-grade pipes wouldn't flex in like that; it's quite a thin wooden set made up as such. (00:04:25)
Revealing mistake: In the tank chase when the police cars and military jeeps crash on each other, just before the camera goes to view the tank, you can see the dummies in the police cars. (01:20:00)
Chosen answer: The real answer, which I believe was explained better in the game or in special features. Regardless, I did hear, is that Ourumov wasn't aiming directly at Alec's head, but to the side and shot the ground behind him. But made it look like he shot him in the head. Alec would have felt the bullet whiz by him.

It has been a while since I watched the movie, and my copy is a VHS... but I believe if you watch very closely you can actually see the bullet hit the ground behind him with a tile breaking. I don't remember for sure if you can, but I seem to remember they did add that little effect.
