Continuity mistake: Brian cuts his entire ear off. The top of his ear reappears a few shots later.
Other mistake: When Peter and the gang are watching TV drinking beers, the TV doesn't have any cables on the back. So how has it got power or a TV signal? This mistake was later referenced in "Back to the Pilot." (00:04:16)
Continuity mistake: Toward the end of this episode, Stewie is at home in the living room talking to Rupert about how depressed he is about being alone. All of a sudden he hears his family come back home, so he gets up from the couch and runs to the window leaving Rupert on the couch. As soon as Stewie moves his head away from the window take notice that Rupert is no longer on the couch.
Petergeist - S4-E26
Other mistake: The Griffins' house disappears when they flee, but when they go to Quagmire's house after, you can see their house when they are at the front door. Quagmire's house is on the right of theirs and the angle of the shot is to the left so it's definitely their house, not Quagmire's other neighbours on his right. (00:16:36)
There's Something About Paulie - S2-E16
Continuity mistake: A mob member at Cleveland's deli offers to help Peter get a new car by stealing the used junker he bought and have insurance get him a new one. The new blue car he receives appears to resemble a Volkswagen Beetle when it first appears, but in the second shot the car's body is more Mercedes-like. (00:05:17)
Road to the North Pole - S9-E7
Other mistake: Cleveland's house, across the street from the Griffins, is absent in the opening overhead shot. Then the scene changes to inside, and we can see his house through the window.
Continuity mistake: Peter, Quagmire, Cleveland and Joe are crawling through the air duct. Joes wheelchair is nowhere in sight yet when they fall to the floor it hits Joe in the head. (00:10:18)
Factual error: If one of a person's parents is an American citizen, that person has citizenship no matter where they are born. Since Thelma was a U.S. citizen, that makes Peter a citizen of the U.S. Also, were there no tax records for Peter since he had been married to Lois?
Other mistake: A bank wouldn't lock someone in the vault all night. Employees make sure customers have left before locking up for the day.
Death Lives - S3-E6
Other mistake: When Peter is dumped into the sea, a ship appears on the right hand side of the screen. However, the ship just appears randomly, it doesn't sail in to the shot, one frame its open water and the next frame it's there. (00:10:20)
Factual error: When Carter puts on the Jack LaLanne record, you can see it has eight individual grooves (or rings) around it. However, records only have one continuous groove.
Character mistake: In "The Drunken Clam", while Quagmire and Joe are playing table tennis, a poster on the wall incorrectly spells "Pawtucket Patriot ale" as "Pawtucket Patrtot ale." (00:00:30)
Continuity mistake: When Peter and the family reach the top of Mt Everest, it shows a long shot of them all by themselves. Cut to Lois talking - another family is suddenly sitting there, and the woman says "we beat you by one hour and 11 minutes." (00:14:00)
Continuity mistake: When Brian and Stewie arrive in Leonardo's atelier we know that they are in Florence for two reasons: because of history and because we see Palazzo Vecchio (which is in Piazza della Signoria, in Florence) outside the window. Then when Stewie and Bertram are fighting with Leonardo's flying machine the city of Florence is wrongly represented as if it was Venice, with the see, the lagoon and the canals.
Revealing mistake: When Brian says "Our tickets were in the bags", half of his left hand is missing for a frame.
Revealing mistake: When Brian asks Peter how much money they're getting from welfare, his neck is drawn over his collar, making his head look like it's disconnected from his body.
Continuity mistake: In the episode "FOX-y Lady" (7-10) it was established that Quagmire was 61 years old. In this episode, Quagmire stated to Meg he was bullied in 1986 and a flashback shows that he was a teenager. So how could Quagmire have been bullied in 1986 as a teenager if he should have been in his 30's at this time? (00:12:15)

Factual error: Near the end when Stewie is being put back and out of Lois's womb, he has no umbilical cord attached to him.

Continuity mistake: When Stewie is born whilst time is reverting, Doctor Hartman says "It's a girl... with a penis and no vagina". Then after Brian restored the flow of time and Stewie is actually born, Doctor Hartman instead says "It's a boy". (00:18:25 - 00:19:55)
And Then There Were Fewer - S9-E1
Continuity mistake: When Mayor West panics, Peter shakes him and he falls against a shelf. The plate on the end of the shelf falls off and smashes on the floor. But when Brian points out the gun is there, it is now the middle plate that fell off and the plate on the end of the shelf is intact. (00:15:25)
Answer: Mort is Jewish, and Walt Disney was said to be antisemitic. So it would make sense in a Disney universe all the inhabitants would share his supposed beliefs.
