Other mistake: While Roy is showing Brice the squad, after Brice annoyingly locks Squad 51's side bay door, he asks, "How about the OB box?" but the subtitles read, "How about the OV box?" Then a minute later Brice says, "Let's look at the OB box," and again the subtitles read "OV box." The OB box is obstetrics supplies, and OV box doesn't exist. Other mistake: When Johnny and Roy are at the house with the three wild, obnoxious boys who terrorized their sitter, Johnny takes her blood pressure with the cuff, while Roy takes her respiration and pulse as he looks at his wrist for the second-hand countdown, but the problem is that Roy isn't even wearing his watch, and the actor has a bizarre look on his face when he looks at his naked wrist. He then tells Johnny that her pulse is 140 and respiration is 35. When the ambulance attendants arrive, Roy is wearing his watch. If he had his watch he should have used it for the pulse and respiration.
Visible crew/equipment: When Roy and Johnny are driving in the squad, while discussing Roy's last phone call from Susan, Johnny's lack of tact, etc., and particularly as they're being dispatched to the "possible maternity case," in every shot facing Johnny pay attention to the window behind the guys, because we can see a crew member's right hand, wearing a ring, who's holding a rope while he's riding on the back of the squad.
Other mistake: When John and Roy treat the 12 year old boy in anaphylactic shock, Early states, "Start IV D5W, 1 milligram epinephrin IV push," and John confirms the order. Then after giving Rampart a new set of vitals which are poor, Early orders, "Add 50 milligrams Benadryl IV push." But when John confirms that order he only says, "1 milligram epinephrine IV push," (which is the earlier order) and Early actually okays John's incorrect repeat of the order, even though John did not confirm the order of "50 milligrams Benadryl."
The Wedsworth-Townsend Act - S1-E1
Visible crew/equipment: During the factory fire, the Battalion Chief tells dispatch over the HT, "Fire through the roof, send me a third alarm," then in the next shot we see Engine 36 arrive, and when the engine comes to a full stop we can see two tripod lighting rigs and stacked film canisters, at the right of the screen.
Visible crew/equipment: On scene at the traffic accident with Paula, after Johnny brings the IV bottle to Roy, it cuts to the overhead shot of the ambulance arriving, and a large reflector screen is visible at the left of the screen.
Audio problem: When Vince and Mike pull the tabletop out from under the car, while Roy is under there with Mrs. Merkle, we hear the audio of Roy loudly saying, "I'm telling you right now, we're gonna have to brace this thing well, this thing will...," but what's oddly funny is that the dialogue is dubbed right over Roy saying, "Wait, wait, let me check her over...," which is a neat trick that he can say both lines at the same time.
Revealing mistake: When Squad 51 leaves Rampart, in the first interior shot facing Roy as John says, "Yeah, I'll tell ya, when it hits ya. Boy when I met Valerie it hit me," through the back window we see movement at the back of the squad. Then in the shot facing Roy as John says, "So that's that... That's exactly how it is," through the back window we see a crew member's face (sideways), he has a mustache (John does not have a mustache), right behind John's head, as he's riding atop the back of the moving squad. Other mistake: After Johnny takes Captain Stanley's BP, when Chet tells the guys, "Animal fat is the root of all evil," and also after John jokes, "How to survive on no meals a day," check out Roy's feet because he's wearing shoes (looks like black sneakers) that are below the ankle, with no socks. Then in a few moments they're all dispatched to the 'possible heart attack' and when they're at the scene, while Roy is sitting with the patient, he's wearing socks and his uniform shoes (their tops are above the ankle) as usual.
Visible crew/equipment: When Chet, Roy, and Johnny are with the mother and child at the scene of the vehicle accident, the reflections of location set array lights, four side-by-side reflector screens with some crew members holding them and the camera/cameraman are all visible in the window and surface of the funeral home hearse, as it arrives and then pulls to the side of the road.

Visible crew/equipment: After Roy and Johnny rescue the worker in the midst of the chemical leak, when Captain Stanley asks the Chief over the HT to send the ambulance, two crew members are seen behind him. When the victim is pulled across the lifeline, as he reaches the engine a crew member, with black hair wearing a white shirt, is seen standing behind the engine. Then, when Johnny hauls himself across the lifeline, just as he reaches the engine we see a crew member, with black hair wearing a white shirt (likely same one as earlier), who arches up then ducks back down behind the engine.
Revealing mistake: When Captain Stanley runs into the burning house looking for Johnny, the floor beside the stairs (under the table) is visible behind him, and there is no rug only wall-to-wall carpet, but when Johnny and Toby fall from the stairs, that area of floor is covered in a rug raised up like a platform, covering the crash mat for the stunt, then the rug vanishes when the guys get Johnny and Toby.
Continuity mistake: When Johnny, Roy, and Captain Stanley are rescuing the people trapped inside Olive View Hospital, they are wearing distinctively marked SCBA tanks on their backs, and when all three firemen exit the doorway, Cap is wearing an entirely different tank, and the tanks that Johnny and Roy are wearing have been switched. Then when Johnny jumps off Engine 82, his tank differs again, and when he walks over to the Battalion Chief it's changed yet again.
Other mistake: At the start of this episode the tones drop and Station 51 is dispatched to a fire and explosion. In this closeup, all the guys come running from the kitchen into the apparatus bay and head for the vehicles, but wait, none of them are actually 51's guys.
Visible crew/equipment: When Engine and Squad 51 are at the cliff with the young unconscious boy, Captain Stanley calls in to dispatch, with Stoker standing beside him. The reflection of the camera crane and location set lights are visible in the side door mirror.
Continuity mistake: When Johnny is writing the 'for sale' sign about his Land Rover, in the wide shot the two phone numbers begin with "853" and "352", but in the closeup a piece of paper has been glued over the phone numbers, and the new numbers now begin with "321" and "749."
Visible crew/equipment: During the fire late at night, after Johnny and Roy run to the burning house, when Captain Stanley tells Chet to get the exposure and later warns to watch for spot fires, the moving shadow of the cameraman/camera is visible on the ground, right beside Captain Stanley's shadow.
Other mistake: Roy uses the Biophone when he tells Brackett the vitals of a woman with cardiac symptoms, and then Johnny uses the landline to tell Brackett that there's "Another victim, female, 8 months pregnant, she's gone into labor." Brackett responds, "The mother's probably had an MI... Any cervix dilation on the daughter?" It is impossible for Brackett to have known that the two women are related, let alone that they're mother and daughter.
Pressure 165 - S5-E7
Other mistake: When Johnny prepares Chef Michel's recipe, he leaves the casserole dish in the oven while Station 51 is dispatched to the structure fire. When he and Roy return, Johnny opens the hot oven door to find it smoking and the dish bubbled over, and even though the oven door is supposedly very hot, Roy places his hand on the inside of that door and actually leans on it like there's absolutely no problem.
Other mistake: When Brackett tells Johnny and Roy to contact the animal shelter via the disaster net, Roy asks the Animal Control guys for their disaster call, and it's given as 3-9-9-7-7-1-1, but in the closeup Roy presses channel 8 then punches in 4-2-1-6-5-3-7 on the scanner. Additionally, this model Regency Monitoradio / Executive Scanner Receiver has 8 channels, so it would have been impossible to press 9, as was stated by Dave.
