Visible crew/equipment: When JP notices the box has been taken out of the statue and hears a noise coming from inside, there's a dramatic shot where the camera swoops from one side of the statue to the other, and you can see the shadow of the cameraman pass over the statue, plain as day. (00:18:34)
Visible crew/equipment: After the clubgoer's head explodes, we see Joey riding in a bus. In the first shot, as the camera moves towards Joey, towards the end of the shot, you can just barely see the reflection of a crew member in the window. He appears to be pulling or steadying the camera rig (probably a dolly camera rig or something similar).
Visible crew/equipment: When Terri is attacked by JP, after she knocks him, there's a shot of her scurrying towards the door. At the bottom left of the screen, you can see a boom mic quite prominently for almost two full seconds. (Seen in the 16:9 Blu-Ray from Arrow Video. Some versions of the film appear to be somewhat cropped, so it may not be visible in all versions.)
Visible crew/equipment: When Joey enters the nightclub at the beginning of the film, you can see the camera's shadow pass over the doorman on the right side of screen as it follows her inside. (Bonus: The doorman also looks directly into the lens for a second.)
Chosen answer: It's a piston. 2 piston rods are jammed through his head and they move powered by something unknown. You can see the crank shaft and part of some sort of cylinder. A piston is part of an internal combustion engine. It's part of his cenobite form since he liked cars.
