Factual error: The nuke ETs are sitting at a RADAR station, not a power output monitoring station. The RADAR wouldn't give any sort of indication of sudden additional power draw on the reactor. (01:12:00)

Time Trax (1993)
Factual error: The scenes leading up to the error show the antagonists driving south from Malibu, CA towards San Diego, but in a shot showing their van driving, there is a U.S. Hwy 79 sign. US Hwy 79 runs from Texas to Kentucky. (00:25:45)

Men of Honour (2000)
Other mistake: In the courtroom scene, when Chief Sunday tells Brashear to stand up, the tenders go to help him up without question, and when Captain Hanks overrides him, saying that he has to stand up himself, even they look confused and surprised. They wouldn't be just some random enlisted sailors doing this, they have been trained in being a tender, and would reasonably know what the policies are. (01:56:50)

Airwolf (1984)
Other mistake: General Butarra holds his pistol to St. John's head asking him where the diamonds are, dry firing it when he doesn't tell him, so the chamber must be empty. He then proceeds to load a magazine into the gun, but never chambers a round, and then proceeds to shoot the radio when it receives a storm alert broadcast. (00:28:40 - 00:29:11)

Ground Zero - S4-E14
Other mistake: When Hawke blows up Yahara's plane, we see that the video feed in the holding cell where Locke, Santini, and Rivers are is actually from a 3rd person perspective and that it continues, but according to previous dialogue (and video up to that point) the camera was installed on the plane, so once Hawke blew the plane up, there should have been no more video feed. Eraser (1996)
Plot hole: In the plane scene where Deguerin is holding a gun to the pilot's head telling him to come around and hit Krueger with the plane, it is pretty evident that the pilot's not in on the scheme, so why wouldn't he tell what happened as soon as he landed, giving away that Deguerin was the bad guy (or at least one of them)? Under Siege (1992)
Factual error: In the last scene, where Chief Ryback is at his captain's funeral, he's in his dress blues, and you can see his rating insignia to see that it is not of a cook (Mess Specialist/MS, now known as "Culinary Specialist/CS"), but that of a Yeoman. While in general, this still matches up with the history of the character (where Krill mentions that he could only be a cook or a yeoman after losing his security clearance for punching out his OIC), he wouldn't be cooking, he'd be doing office work. Star Trek: Deep Space Nine (1993)
Factual error: When Major Kira was introducing Lieutenant Commanders Dax and Worf to the arriving Trill scientists, she introduces them as Commander Dax and Worf. While in informal conversation, they could be addressed as Commander, when making formal introductions, the person making the introductions would use their full proper rank. This is an error that no ranking officer would make, especially not twice. (00:03:10)
Suggested correction: And when was this established? If you are basing this error on modern American military protocols, there is no reason Starfleet in the 24th century would follow the same rules and traditions.
Yes, although it's not just American military introductions, it's pretty standard in most militaries to use full rank when making formal introductions, which has been followed for the most part by the writers, with other introductions that have happened onscreen.

Deep Cover (1992)
Other mistake: In the club bathroom scene, Ivy has just finished urinating when Russell Stevens Jr./John Hull walks into the bathroom, where Ivy taunts John for a bit, and then Ivy proceeds to urinate on his leg and shoes. He wouldn't have any urine left to do that. (00:44:15)

The Nanny (1993)
Shopaholic - S3-E6
Factual error: In the last scene, Fran has a soda cup from In-N-Out, which at the time, of the show, only had one restaurant out of the state of California, in the state of Nevada. Even today (2020), they don't have any stores east of Texas.

Cars 3 (2017)
Factual error: In the end credits, some of the vintage advertisements have 770 area code phone numbers, which while valid for Atlanta, Georgia and surrounding areas now, it didn't exist until 1995 when it was assigned. The numbers should have had either a 404 or 912 area code since those 2 area codes covered all of Georgia until 1992 when area code 706 was implemented.

The Equalizer (2014)
Factual error: In the big battle scene at the Home Mart store after Robert turns the power off to the building, you see Robert grabbing welding canisters. When Robert has Ralphie turn the power back on, it cuts to one of the bad guys in the break-room, and a digital microwave starts counting down. A digital microwave could not have any memory of any prior cook settings, let alone any way to set it, since the power was already out. (01:55:40 - 01:56:35)
