Continuity mistake: When Buffy is fighting the three Hellhounds outside the school, she kills one of them and the other two enter the school, having heard some music playing from inside. But when Buffy follows them inside, the music has stopped.

Other mistake: At the end the library that was rigged with explosives blows up. But the school's main entrance and the clock tower blow up as well, even though they are not rigged with explosives.

Other mistake: When we first see one of the Hellhounds at the clothes store that Cordelia works at, it jumps through the window, smashing the glass, and then attacks a customer who's wearing a tuxedo. But the Hellhound attacks the customer several seconds after it smashes through the window, giving the customer a chance to escape. But the customer just stands there unaware of the Hellhound, having not heard the window being smashed.

Continuity mistake: Seeing as how the original sets for the ship's interior from "Firefly" were demolished after the show got cancelled, the interior had to be re-built from scratch for Serenity. Because of this, there are some noticeable differences between the two. For example: The long corridor that holds the entrances to the crew's sleeping quarters is a lot wider. The hand rail on the stairs leading from the infirmary to the cargo bay is missing. The back wall of the cargo bay, the entire engine room (including Kaylee's hammock), some of the table settings in the kitchen and the furniture in the lounge area are all different to how they were in "Firefly".

Revealing mistake: On Miranda there is a shuttle crashed into the side of a building, but there are no burn marks or a trail from where it had crash landed. The shuttle couldn't possibly have crashed into the building horizontally without crashing diagonally/vertically first, landing horizontally and then sliding into the building. Joss Whedon even states in the director's commentary that some burn marks would have been nice but it only occurred to him about 14 months after it was filmed. (01:17:15)

Continuity mistake: During River's first fight scene she climbs some stairs. She then does two forward flips down the stairs. As she lands, her hair is raised from doing the flips but in the immediate shot her hair is down again. (00:34:55)

Revealing mistake: When the Doctor travels back in time after being shot by the stone Dalek, falls down the stairs and tells himself that he has 12 minutes to live we never see either of the 2 Doctors' faces onscreen at the same time, and if you look at the hair on the Doctor that's dying it is completely different to Matt Smith's. That's because it is Gordon Seed, Matt Smith's stunt double.

Revealing mistake: Near the end of the episode when the Mayor releases the vampire from his cage and lets him cut his head in half, you can tell that the Mayor's head is a still image when it fixes itself. The expression on his face doesn't change at all and looks completely unrealistic.

Revealing mistake: When evil Willow licks good Willow's neck the neck that you see is a lot longer than either of theirs. Since both Willows' faces cannot be filmed at the same time a body double must have been used for this shot.

Revealing mistake: Right at the beginning of the episode, Buffy is sat outside with Willow and Willow is levitating a pencil. The pencil is obviously CGI as it stands out from the rest of the picture and doesn't cast a shadow. It also disappears from view whenever there is a side-angle shot of Willow.

Revealing mistake: Near the end when the good Willow says goodbye to the evil Willow and hugs her, there is a shot of the back of the good Willow's head and the evil Willow's face. You can tell that the good Willow is a body double wearing a wig - there's something circular near her scalp, possibly an elastic band.

Revealing mistake: Both the good and evil Willows' faces are barely seen onscreen at the same time due to them being played by the same actress, but when they occasionally are the good Willow stands out from the picture more than the evil Willow. She must have been filmed separately from the good Willow and edited in later.

Plot hole: When Buffy enters the house to rescue her mother she uses a stake to stop the front door from closing. A vampire then grabs it and the door closes. Why did Buffy use a stake knowing that there was a vampire there when she was almost killed by a vampire with her own stake earlier in the episode because Giles had been injecting her with a muscle relaxant?
