Continuity mistake: When the buzz droid climbs on Anakin's shuttle it stands in front of R2-D2 with a rotating arm with claws. At first the arm is rotating with the claws out. When it cuts from behind the droid you can see the claws are clenched. (00:06:25)
Audio problem: In the beginning of the scene in the Opera House, the audience is cheering and clapping, but in the two shots where the audience is shown, all of them are sitting still with their hands in their lap. (00:41:20)
Continuity mistake: After Darth Sidious kills Mace Windu, Anakin drops his lightsaber to the floor as he sits down. During the rest of the scene, the saber changes position on the floor twice. (01:13:20)
Continuity mistake: The position and existence of the dead bodies in the control centre on Mustafar keep changing between shots, especially during the duel between Anakin and Obi-Wan.
Continuity mistake: When Obiwan and Anakin are fighting on the narrow walkway, lava splashes over it and causes that portion of the building to fall into the magma river. As it is falling, there is a significant amount of lava on the severed end of the walkway, which ends up as the "top" of the tower once floating on the river. For the rest of the scene this lava is gone, and none is shown falling on or by Obi-Wan and Anakin as they scramble along the side of the tower. (01:55:45)
Continuity mistake: When Obi-Wan and Anakin are fighting on the table in the control centre, they are standing/crouching in a different position from the previous shot after every time they kick each other. For example, when Obi-Wan kicks Anakin for the second time, as the shot ends, both of his feet are on the table, but in the next shot, one of his feet are up again.
Continuity mistake: In the scene where the twins are born, there are two shots where Padmé is stroking her son's (Luke) cheek. As the second shot ends, she's still stroking his cheek, but in the next shot, she is no longer touching him. (02:00:45)
Continuity mistake: After Anakin has been turned down as a Master in the council, he is walking with Obi-Wan. While they are walking you can count them passing at least five windows. When it cuts to Obi-Wan saying "Anakin, I'm on your side, I didn't want to put you in this situation", you can see, by looking at the sun shining through the windows, that they have only passed three windows. Continuity mistake: The scar over Anakin's eye changes throughout the movie. For example, when he is talking to Yoda about his premonition, the scar is different from the previous scene - the part below his eye moves slightly down and lengthens.
Continuity mistake: On Utapau when Obi-Wan gets off the creature before jumping down to Grievous, he pats the creature on its head. When it cuts his hand is nowhere near the creature. (00:54:45)
Continuity mistake: When C-3PO tells Padme that Anakin hasn't returned from the Jedi Temple and says he will be alright, you see Padme holding her right hand on her left waist. When it cuts she suddenly has her hand in front of her stomach and moves it back to her waist again. (01:20:40)
Continuity mistake: When Obi-Wan and Anakin are fighting Griveous' droids, Palpatine is being dragged against the door. When Griveous says "Stay at your stations" it cuts and we can briefly see Palpatine standing still in the left corner. (00:18:55)
Continuity mistake: During Anakin and Padmé's conversation on Mustafar, in the first shot, they are standing near some sort of container. They continue to stand in the same place in the following shots. When Anakin sees Obi-Wan and becomes furious, both of them take several steps away from the container, but when Anakin says, "You brought him here to kill me.", they are standing near it again. In the following shots, it is nowhere to be seen. (01:39:35 - 01:41:55)
Continuity mistake: In the scene where Obi-Wan and Yoda find the dead Jedi younglings, in the first shot, there are no bodies on the stairs. But when Yoda says, "Killed not by clones, this Padawan.", you can see a dead body on the bottom of the stairs. (01:30:40)
Continuity mistake: When Anakin and Padmé are talking on the couch after Anakin is appointed to the Jedi council, they keep changing position between shots, depending on the angle of the camera. The easiest way to see this is to watch the red cloth hanging behind them on the couch. In some shots it is behind Padmé, in other shots, it is behind Anakin.
Audio problem: When the young Jedi is killed and Bail Organa shouts "No." and climbs into his ship, shots can be heard but no lasers can be seen. (01:21:35)
Audio problem: In the scene where Ki-Adi-Mundi is killed on Mygeeto, he yells, "Come on", but the words don't match the movement of his lips. (He moves his mouth like the lips of a human, even though he's an alien.). (01:18:05)
Continuity mistake: In the scene where Luke and Leia are born, the blankets under the babies are arranged differently between several shots.
Continuity mistake: When Bail Organa hands Leia to his wife, Breha Organa, the floor around Breha is covered in shadow. In the close-up of Leia being held, there are no shadows, and the design of the floor is different. (02:07:20)
Continuity mistake: When Anakin tells Padme he's found a way to save her, she says "Is that what's bothering you" while placing her hand on his chest and pulling it away again. When it cuts her hand is still on his chest. (00:51:30)
Chosen answer: Powerful force users seem to have some degree of control over their bodies even after death. In the later series, Luke's wife Mara Jade Skywalker only allows her body to disappear when her killer, and nephew Jacen Solo arrives at her funeral as a clue. Thus it appears that a powerful force user can simply choose if they wish their body to disappear.
